Description
Overview of the medicine
Deflazacort is a glucocorticoid used for its anti-inflammatory and immunosuppressant effects. It's often prescribed for various conditions where corticosteroids are indicated, including Duchenne muscular dystrophy.
Uses & Indications
Dosage
Adults
Dosage varies significantly based on indication and patient response. For Duchenne muscular dystrophy, 0.9 mg/kg/day, typically ranging from 6 mg to 36 mg daily. For other conditions, initial doses typically range from 6-18 mg/day, adjusted based on response.
Elderly
Use with caution; lower initial doses may be considered.
Renal_impairment
No specific dose adjustment generally required, but monitor patients closely.
How to Take
Take orally, usually once daily with or without food. It is recommended to take it with food to minimize gastrointestinal upset. Swallow the tablet whole; do not crush or chew.
Mechanism of Action
Deflazacort is a corticosteroid that acts by binding to glucocorticoid receptors, modulating gene expression, and thus exerting anti-inflammatory and immunosuppressive effects. It has a lower propensity for bone demineralization and glucose intolerance compared to some other corticosteroids.
Pharmacokinetics
Onset
Varies based on indication, generally within hours to days.
Excretion
Primarily via urine (about 70%) and feces (about 30%) as metabolites.
Half life
The active metabolite (21-desacetyl deflazacort) has a half-life of 1.1 to 1.9 hours.
Absorption
Well absorbed orally. Peak plasma concentrations are reached within 0.5 to 2 hours.
Metabolism
Rapidly converted to its active metabolite, 21-desacetyl deflazacort, primarily by esterases. Further metabolized in the liver.
Side Effects
Contraindications
- •Systemic fungal infections
- •Known hypersensitivity to deflazacort or any component
- •Live virus vaccines during corticosteroid therapy
Drug Interactions
Anticoagulants
May alter anticoagulant effects, requiring dose adjustment.
Diuretics (e.g., furosemide)
Increased risk of hypokalemia.
Non-steroidal anti-inflammatory drugs (NSAIDs)
Increased risk of gastrointestinal ulceration.
CYP3A4 inhibitors (e.g., ketoconazole, ritonavir)
May increase deflazacort levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is unlikely to cause severe toxicity. Chronic overdose can lead to signs of hypercorticism (e.g., Cushing's syndrome features). Management is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs potential risk. Deflazacort is excreted in breast milk; avoid breastfeeding or discontinue the drug.
