Description
Overview of the medicine
Xalcort 6 mg Tablet contains Deflazacort, which is a corticosteroid. It is used for its anti-inflammatory and immunosuppressive effects to treat a variety of conditions, including allergic disorders, asthma, rheumatic disorders, skin conditions, blood disorders, eye disorders, and certain cancers. It works by reducing inflammation and modifying the body's immune response.
Uses & Indications
Dosage
Adults
Dosage varies significantly based on the condition being treated and patient response. Typically ranges from 6 mg to 90 mg daily, divided into single or multiple doses. For Duchenne muscular dystrophy, typical dosage is 0.9 mg/kg/day.
Elderly
No specific dose adjustment for elderly patients, but caution is advised due to increased risk of side effects.
Renal_impairment
No specific dose adjustment is usually required for renal impairment, but careful monitoring is recommended.
How to Take
Take Xalcort tablet orally, with or without food, as directed by your doctor. It can be taken with food to minimize stomach upset.
Mechanism of Action
Deflazacort is a glucocorticoid that acts by binding to specific glucocorticoid receptors inside cells, altering gene expression. This leads to the synthesis of anti-inflammatory proteins and inhibition of pro-inflammatory mediators, thereby reducing inflammation and suppressing the immune system.
Pharmacokinetics
Onset
Within hours of administration, peak effects typically within 1-2 hours for active metabolite.
Excretion
Mainly excreted via the kidneys (primarily as metabolites) and in feces.
Half life
The active metabolite has a half-life of 1.1-1.9 hours.
Absorption
Well absorbed orally, rapidly converted to its active metabolite, 21-desacetyl deflazacort.
Metabolism
Primarily metabolized in the liver to the active metabolite (21-desacetyl deflazacort) by esterases, followed by further metabolism.
Side Effects
Contraindications
- •Systemic fungal infections
- •Hypersensitivity to deflazacort or any component of the formulation
- •Administration of live or live-attenuated vaccines during corticosteroid therapy
Drug Interactions
NSAIDs
Increased risk of gastrointestinal bleeding and ulceration.
Warfarin
Altered anticoagulant effect, monitor INR closely.
Antidiabetics
Corticosteroids may increase blood glucose levels, requiring dose adjustment of antidiabetic agents.
CYP3A4 Inducers (e.g., Phenytoin, Rifampicin)
May decrease deflazacort levels.
CYP3A4 Inhibitors (e.g., Ketoconazole, Erythromycin)
May increase deflazacort levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is rare. Symptoms may include exaggerated side effects such as fluid retention, high blood pressure, and hyperglycemia. Treatment is symptomatic and supportive; gradual withdrawal is necessary for chronic overdose.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Small amounts may pass into breast milk; caution advised during lactation.
