Description
Overview of the medicine
Xalcort 6 mg Suspension contains Deflazacort, a corticosteroid used to treat various inflammatory and autoimmune conditions, including rheumatic disorders, asthma, and certain skin conditions. It works by reducing inflammation and suppressing the immune system.
Uses & Indications
Dosage
Adults
Initial dose typically 6-90 mg daily, adjusted according to response and nature of the disease. Often 6-18 mg daily for maintenance. For DMD, 0.9 mg/kg/day.
Elderly
No specific dose adjustment generally required, but caution should be exercised due to increased risk of side effects.
Renal_impairment
No specific dose adjustment required, but use with caution.
How to Take
For oral administration. Shake the bottle well before use. Can be taken with or without food, but taking with food may reduce gastrointestinal upset. Use the provided measuring device for accurate dosing.
Mechanism of Action
Deflazacort is a glucocorticoid with anti-inflammatory and immunosuppressive properties. It binds to specific glucocorticoid receptors in the cytoplasm, leading to the formation of a steroid-receptor complex. This complex translocates into the nucleus, modulating gene expression by activating or inhibiting the transcription of various genes involved in inflammatory and immune responses. This results in decreased production of inflammatory mediators, reduced leukocyte migration, and suppression of immune cell function.
Pharmacokinetics
Onset
Variable, depending on the condition being treated, but generally within hours to days for anti-inflammatory effects.
Excretion
Primarily excreted via urine (about 70%) and feces (about 30%) as metabolites.
Half life
The active metabolite (21-desacetyl deflazacort) has a plasma half-life of 1.1-1.9 hours.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 1.5-2 hours.
Metabolism
Rapidly metabolized in the liver to its active metabolite, 21-desacetyl deflazacort.
Side Effects
Contraindications
- •Systemic fungal infections
- •Hypersensitivity to deflazacort or any components of the suspension
- •Administration of live or live-attenuated vaccines during treatment (unless otherwise advised by physician)
Drug Interactions
NSAIDs
Increased risk of gastrointestinal ulceration.
Oral anticoagulants
May alter anticoagulant effects (monitor INR).
Insulin/Oral hypoglycemics
May increase blood glucose levels, requiring adjustment of antidiabetic therapy.
Diuretics (e.g., Thiazides, Loop diuretics)
May enhance potassium depletion.
CYP3A4 Inhibitors (e.g., Ketoconazole, Erythromycin, Grapefruit juice)
May increase deflazacort levels, requiring dose adjustment.
CYP3A4 Inducers (e.g., Rifampicin, Phenytoin, Carbamazepine, Barbiturates)
May decrease deflazacort levels, requiring dose adjustment.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children. Do not freeze.
Overdose
Symptoms of acute overdose are unlikely to occur with Deflazacort due to its unique pharmacokinetic profile. However, chronic overdose may lead to Cushingoid features, muscle weakness, and adrenal suppression. Treatment is supportive; gradual withdrawal is usually recommended.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit outweighs the potential risk to the fetus. Corticosteroids are excreted in breast milk; caution should be exercised when administered to a nursing mother. Consult a doctor.
