Description
Overview of the medicine
Cefixime is a broad-spectrum cephalosporin antibiotic used to treat various bacterial infections including those of the respiratory tract, urinary tract, and ear, nose, and throat.
Uses & Indications
Dosage
Adults
The usual adult dose is 100-200 mg orally once daily or in two divided doses (e.g., 100 mg twice daily) for 7-14 days depending on the severity and type of infection. For uncomplicated gonorrhea, a single 400 mg dose is typically given (not applicable for 100 mg capsule for single dose).
Elderly
No specific dosage adjustment is necessary for elderly patients unless there is severe renal impairment. Monitor renal function.
Renal_impairment
For patients with creatinine clearance less than 60 mL/min, the dose should be adjusted. For CrCl 21-60 mL/min, the dose should be reduced to 75% of the standard dose. For CrCl < 20 mL/min, the dose should be reduced to 50% of the standard dose.
How to Take
Take orally with or without food. To minimize gastric upset, it can be taken with food. Swallow the capsule whole with water.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Within 1-2 hours (therapeutic effect).
Excretion
Primarily renal excretion (approximately 50% unchanged in urine) and some biliary excretion. About 10% is excreted via bile.
Half life
Average 3-4 hours, can be longer in renal impairment.
Absorption
Approximately 40-50% orally absorbed. Peak serum concentrations are generally achieved between 2-6 hours after administration.
Metabolism
Not extensively metabolized; some hepatic metabolism.
Side Effects
Contraindications
- Hypersensitivity to cefixime or other cephalosporins.
- History of serious hypersensitivity reactions to penicillin.
Drug Interactions
Nifedipine
May increase cefixime absorption.
Carbamazepine
Concomitant administration with cefixime has been shown to increase carbamazepine levels.
Warfarin and other anticoagulants
May increase prothrombin time (INR) and the risk of bleeding. Close monitoring of coagulation parameters is recommended.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Gastric lavage may be indicated; otherwise, no specific antidote exists. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Management should be supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use in pregnancy only if clearly needed. Cefixime is excreted in small amounts in breast milk; use with caution in lactating mothers.
