Description
Overview of the medicine
Zedoxim 20 mg Pediatric Drop is a broad-spectrum antibiotic containing Cefpodoxime proxetil, a third-generation cephalosporin. It is used to treat various bacterial infections in children, including respiratory tract infections, ear infections, and skin infections.
Uses & Indications
Dosage
Adults
For infants and children (2 months to 12 years): 5 mg/kg body weight every 12 hours. Maximum dose 200 mg/day. For exact dosage, consult a physician based on the child's weight and infection severity.
Elderly
Not applicable for this pediatric formulation.
Renal_impairment
Dosage adjustment is necessary for pediatric patients with severe renal impairment (creatinine clearance <30 mL/min); consult physician for specific instructions.
How to Take
Administer orally with food to enhance absorption. Use the provided dropper for accurate measurement. Shake well before use.
Mechanism of Action
Cefpodoxime proxetil is a prodrug that is de-esterified in vivo to the active metabolite cefpodoxime. Cefpodoxime is a beta-lactam antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Approximately 1-3 hours
Excretion
Primarily excreted unchanged in the urine.
Half life
Approximately 2-3 hours
Absorption
Well absorbed from the gastrointestinal tract after oral administration. Bioavailability is increased with food.
Metabolism
Primarily hydrolyzed to the active compound cefpodoxime in the intestinal wall. Minimal hepatic metabolism.
Side Effects
Contraindications
- Hypersensitivity to cefpodoxime or any other cephalosporin antibiotics
- Patients with a history of severe hypersensitivity reactions to penicillin (due to potential cross-reactivity)
Drug Interactions
Probenecid
Increases cefpodoxime plasma levels by decreasing renal excretion.
Antacids/H2-receptor antagonists
May decrease absorption of cefpodoxime, leading to reduced efficacy. Administer cefpodoxime at least 2 hours before or after these agents.
Oral Anticoagulants (e.g., Warfarin)
May increase the anticoagulant effect, requiring close monitoring of INR.
Storage
Store dry powder at room temperature (below 30°C), protected from light and moisture. After reconstitution, store the suspension in a refrigerator (2-8°C) and discard any unused portion after the specified period (e.g., 7 or 14 days).
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, and diarrhea. In case of acute overdose, gastric lavage or induced emesis may be performed. Hemodialysis and peritoneal dialysis are not effective in removing cefpodoxime from the body.
Pregnancy & Lactation
Cefpodoxime is generally considered safe during pregnancy (Category B), but use only if clearly needed. Excreted in breast milk in small amounts; use with caution in nursing mothers and monitor for adverse effects in the infant.
