Overview of the medicine
Cefixime is an oral third-generation cephalosporin antibiotic used to treat a variety of bacterial infections, including those of the respiratory tract, urinary tract, and ear, nose, and throat.
Uses & Indications
Dosage
Adults
Adults and children over 10 years or weighing more than 50 kg: 200 mg once daily or 100 mg twice daily. For severe infections or uncomplicated gonorrhea, dose may be increased to 400 mg once daily.
Elderly
No dosage adjustment needed for elderly patients with normal renal function.
Renal_impairment
Creatinine clearance 21-60 mL/min: 75% of standard dose. Creatinine clearance <20 mL/min: 50% of standard dose.
How to Take
Cefixime capsules can be taken with or without food. To minimize gastrointestinal upset, it is often recommended to take with food.
Mechanism of Action
Cefixime exerts its bactericidal effect by interfering with the synthesis of the bacterial cell wall. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby inhibiting the third and final stage of bacterial cell wall synthesis. Cell lysis is then mediated by bacterial cell wall autolytic enzymes.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 2-6 hours after administration.
Excretion
Approximately 50-60% excreted unchanged in urine over 24 hours, about 10% via bile.
Half life
Approximately 3-4 hours.
Absorption
Approximately 40-50% absorbed from the gastrointestinal tract, food delays absorption but does not significantly affect overall absorption.
Metabolism
Not extensively metabolized; primarily excreted unchanged.

Contraindications
