Overview of the medicine
Zerotil 125 mg Suspension contains Cefuroxime Axetil, a second-generation cephalosporin antibiotic, used to treat a wide range of bacterial infections in children and adults. It is effective against various Gram-positive and Gram-negative bacteria.
Uses & Indications
Dosage
Adults
Not typically used for adults in 125 mg suspension form; adult dose is usually 250 mg or 500 mg tablets. For children: 125 mg twice daily for most infections. For otitis media or more severe infections: 250 mg twice daily.
Elderly
Dosage adjustments may be necessary based on renal function.
Renal_impairment
Dose reduction is necessary in patients with significant renal impairment (CrCl <30 mL/min).
How to Take
Oral administration. Shake the bottle well before each use. Can be taken with food to enhance absorption and reduce gastrointestinal upset. Complete the full course of treatment as prescribed, even if symptoms improve.
Mechanism of Action
Cefuroxime Axetil acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan units, leading to the inhibition of cell wall synthesis and eventual bacterial cell lysis.

Plasma concentrations peak within 2-3 hours after oral administration.
Excretion
Primarily excreted unchanged by the kidneys via glomerular filtration and tubular secretion.
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract and hydrolyzed by esterases to cefuroxime. Food enhances absorption.
Metabolism
Cefuroxime itself is not metabolized in the body. Cefuroxime axetil is hydrolyzed to cefuroxime in the intestinal mucosa and blood.
Side Effects
Contraindications
- •Known hypersensitivity to cefuroxime, other cephalosporins, or any component of the formulation
- •History of severe hypersensitivity reactions to any beta-lactam antibiotic (e.g., penicillin)
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin (monitor INR).
Probenecid
Increases cefuroxime plasma concentrations and prolongs its half-life by reducing renal tubular secretion.
Oral Contraceptives
May reduce the efficacy of oral contraceptives (though evidence is limited, advise additional contraception).
Antacids/H2 Blockers/PPIs
May reduce the bioavailability of cefuroxime axetil by increasing gastric pH. Cefuroxime axetil should be taken at least 1-2 hours before or after these agents.
Storage
Store dry powder below 30°C, protected from light and moisture. Keep the reconstituted suspension in a refrigerator (2-8°C) and discard any unused portion after 7-10 days.
Overdose
Symptoms of overdose include convulsions, encephalopathy, and other neurological sequelae. Management is supportive. Hemodialysis and peritoneal dialysis can effectively remove cefuroxime from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of impaired fertility or harm to the fetus. Use during pregnancy only if clearly needed. Cefuroxime is excreted in breast milk in small amounts; use with caution during lactation, monitor infant for diarrhea, candidiasis.
