Overview of the medicine
Zerotil 250 mg tablet contains Cefuroxime Axetil, a second-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Most infections: 250 mg twice daily. Severe infections or bronchitis/pneumonia: 500 mg twice daily. Gonorrhea: 1000 mg as a single dose.
Elderly
No special dosage recommendations for elderly patients with normal renal function.
Renal_impairment
Dosage adjustment may be necessary for patients with severe renal impairment (CrCl < 30 mL/min).
How to Take
Oral administration. Take with food for optimal absorption. Tablets should not be crushed.
Mechanism of Action
Cefuroxime Axetil is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). This leads to osmotic instability and lysis of bacterial cells.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved in approximately 2-3 hours after a meal.
Excretion
Primarily excreted unchanged by the kidneys via glomerular filtration and tubular secretion.
Half life
Approximately 1.2-1.6 hours.
Absorption
Well absorbed from the gastrointestinal tract after oral administration, with enhanced absorption when taken with food. Bioavailability is approximately 37% when taken without food and 52% when taken with food.
Metabolism
Cefuroxime axetil is rapidly hydrolyzed by esterases to cefuroxime in the intestinal mucosa and blood. It is not significantly metabolized further in the liver.
Side Effects
Contraindications
- •Hypersensitivity to cefuroxime, any other cephalosporin, or any of the excipients.
- •Previous immediate and/or severe hypersensitivity to penicillin or any other type of beta-lactam antibacterial agent.
Drug Interactions
Warfarin
Increased INR has been reported in patients taking cefuroxime and warfarin concurrently.
Probenecid
May increase cefuroxime levels due to decreased renal tubular secretion.
Oral contraceptives
May reduce the efficacy of oral contraceptives.
Antacids/H2-receptor antagonists
May reduce the bioavailability of cefuroxime axetil.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include cerebral irritation leading to convulsions. Management is supportive, including activated charcoal and gastric lavage if ingested recently. Hemodialysis can reduce serum levels.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Excreted in breast milk in small amounts; use with caution during lactation.
