Overview of the medicine
Cefuroxime is a second-generation cephalosporin antibiotic used to treat various bacterial infections including respiratory tract infections, urinary tract infections, and skin and soft tissue infections.
Uses & Indications
Dosage
Adults
750 mg IV/IM every 8 hours for most infections; 1.5 g IV every 8 hours for severe infections or meningitis. Surgical prophylaxis: 1.5 g IV/IM pre-surgery, followed by 750 mg every 8 hours for up to 24-48 hours.
Elderly
No specific dose adjustment needed unless severe renal impairment. Refer to renal impairment guidelines.
Renal_impairment
Dose reduction required based on creatinine clearance. For CrCl 10-20 mL/min: 750 mg every 12 hours. For CrCl <10 mL/min: 750 mg every 24 hours. Hemodialysis: 750 mg at the end of each dialysis session.
How to Take
Administer intravenously (slow injection over 3-5 minutes or infusion over 30-60 minutes) or intramuscularly (deep injection into a large muscle mass). The powder should be reconstituted with sterile water for injection according to manufacturer's instructions.
Mechanism of Action
Cefuroxime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.

Excretion
Primarily excreted unchanged in the urine via glomerular filtration and tubular secretion.

Side Effects
Contraindications

Drug Interactions
Probenecid
Increases cefuroxime plasma concentrations and prolongs its half-life by reducing renal tubular secretion.
Aminoglycosides
Concurrent use may increase the risk of nephrotoxicity. Monitor renal function closely.
Oral anticoagulants
May potentiate the anticoagulant effect. Close monitoring of INR is recommended.
Storage
Store the dry powder below 30°C in a dry place, protected from light. Reconstituted solution is stable for 24 hours when stored at 2-8°C (refrigerator) or for up to 5 hours at 25°C.
Overdose
Symptoms of overdose may include cerebral irritation leading to convulsions. Management is supportive, and hemodialysis may be considered in severe cases to remove cefuroxime from the body.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnancy only if clearly needed after careful risk-benefit assessment. Cefuroxime is excreted in breast milk in small amounts; use with caution during lactation and monitor the infant for gastrointestinal disturbances (e.g., diarrhea) or fungal infections.
