Description
Overview of the medicine
Brodil 2 mg Tablet contains Bromhexine, a mucolytic agent used to thin and loosen mucus in the airways, making it easier to cough up. It is indicated for the treatment of respiratory conditions associated with excessive or thick mucus.
Uses & Indications
Dosage
Adults
Adults and adolescents over 12 years: Typically 8 mg (4 tablets of 2 mg) three times daily. In some severe cases, up to 16 mg (8 tablets of 2 mg) three times daily may be prescribed under medical supervision.
Elderly
No specific dose adjustment is required for the elderly, but caution is advised in those with severe renal or hepatic impairment. Dose reduction may be necessary.
Renal_impairment
In cases of severe renal or hepatic impairment, the dosage should be reduced or the dose interval prolonged as per physician's advice due to potential accumulation of bromhexine or its metabolites.
How to Take
Take Brodil 2 mg Tablet orally with water, with or without food. It is recommended to drink plenty of fluids while taking this medication to enhance its mucolytic effect.
Mechanism of Action
Bromhexine depolymerizes mucopolysaccharides within the bronchial secretions, increases serous mucus production, and reduces mucus viscosity. This action facilitates the removal of phlegm from the respiratory tract by improving mucociliary clearance.
Pharmacokinetics
Onset
Mucolytic effect typically begins within 30 minutes of oral administration.
Excretion
Mainly excreted renally (about 90%) as metabolites. Only a small fraction is excreted unchanged.
Half life
The terminal elimination half-life is approximately 15-20 hours, primarily due to extensive tissue distribution and slow release from tissues.
Absorption
Bromhexine is rapidly and almost completely absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are reached within approximately 1 hour.
Metabolism
Undergoes extensive first-pass metabolism in the liver, primarily via hydroxylation and subsequent conjugation to glucuronides. One of the main active metabolites is Ambroxol.
Side Effects
Contraindications
- Hypersensitivity to bromhexine or any other component of the formulation.
- Patients with active peptic ulcer or a history of recurrent peptic ulceration (relative contraindication).
Drug Interactions
Antitussives
Avoid co-administration with antitussives (cough suppressants) as this may lead to a dangerous build-up of secretions due to the impaired cough reflex.
Antibiotics (e.g., Amoxicillin, Erythromycin, Cefuroxime)
Concomitant administration of bromhexine with certain antibiotics (e.g., amoxicillin, erythromycin, cefuroxime) may lead to increased penetration of the antibiotic into the broncho-pulmonary secretions, which can be therapeutically beneficial.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and may include gastrointestinal disturbances like nausea, vomiting, diarrhea, and abdominal pain. Treatment involves gastric lavage if recent ingestion and symptomatic supportive therapy. No specific antidote is known.
Pregnancy & Lactation
Pregnancy Category C. Bromhexine should only be used during pregnancy if the potential benefit justifies the potential risk to the fetus. It is not recommended during the first trimester. Limited data indicate bromhexine may be excreted in breast milk; therefore, caution is advised during breastfeeding. Consult a doctor.
