Description
Overview of the medicine
Brodil 200 mcg Inhalation Capsule contains Formoterol Fumarate, a long-acting beta-2 adrenergic agonist (LABA). It is used for the long-term maintenance treatment of asthma and chronic obstructive pulmonary disease (COPD) to improve lung function and reduce exacerbations. Please note that 200 mcg is a substantially higher dose than typically recommended for Formoterol and should only be used under strict medical supervision if prescribed.
Uses & Indications
Dosage
Adults
The typical dose for Formoterol Fumarate in inhalation capsules for asthma and COPD is 6-12 mcg twice daily. A 200 mcg dose is significantly higher than usual clinical practice and its administration should be strictly as per physician's instruction, considering the patient's specific condition and tolerance. Use of such a high dose needs careful monitoring for adverse effects.
Elderly
No specific dose adjustment is required for elderly patients, but caution is advised due to potential comorbidities and polypharmacy. Follow physician's instructions, especially for this high strength.
Renal_impairment
No dose adjustment required for renal impairment. However, as with all drugs, monitor closely.
How to Take
Brodil 200 mcg Inhalation Capsule is for inhalation use only, with a dry powder inhaler device (e.g., rotohaler, revolizer). The capsule must not be swallowed. Read the instruction leaflet provided with the inhaler device carefully for proper technique.
Mechanism of Action
Formoterol selectively stimulates beta-2 adrenergic receptors in the bronchial smooth muscle, leading to relaxation of the muscle and bronchodilation. This action helps to widen the airways, making breathing easier. It also inhibits the release of inflammatory mediators from mast cells in the lung.
Pharmacokinetics
Onset
Within 1-3 minutes
Excretion
Approximately 10% of the dose is excreted unchanged in urine, with metabolites primarily excreted via urine (60%) and feces (30%).
Half life
Plasma elimination half-life is approximately 10-17 hours; lung tissue half-life is longer.
Absorption
Rapidly absorbed from the lungs into systemic circulation. Peak plasma concentrations are reached within 10-15 minutes.
Metabolism
Primarily metabolized in the liver via direct glucuronidation and O-demethylation, followed by glucuronidation.
Side Effects
Contraindications
- Hypersensitivity to Formoterol Fumarate, any other beta-2 agonist, or any component of the formulation.
- Acute bronchospasm or status asthmaticus (not for rescue relief).
- Patients with severe milk protein allergy (if formulation contains lactose).
Drug Interactions
Beta-blockers
May reduce the effect of Formoterol and cause severe bronchospasm. Avoid concomitant use.
Corticosteroids
May potentiate the hyperglycemic effect and hypokalemia.
QT-prolonging drugs
Increased risk of ventricular arrhythmias with drugs like quinidine, disopyramide, procainamide, phenothiazines, antihistamines (e.g., terfenadine, astemizole), macrolides (e.g., erythromycin), tricyclic antidepressants.
Other sympathomimetics
May potentiate the side effects of Formoterol.
Diuretics (non-potassium sparing)
May potentiate hypokalemia, increasing the risk of arrhythmias.
Xanthine derivatives (e.g., Theophylline)
May potentiate hypokalemia.
Storage
Store in a cool, dry place below 30°C. Protect from moisture and direct light. Keep out of reach of children.
Overdose
Symptoms of overdose include tremor, headache, palpitations, tachycardia, cardiac arrhythmias, hypokalemia, hyperglycemia, and nausea. Management is primarily supportive. Beta-blockers (cardioselective) may be considered, but with extreme caution due to risk of bronchospasm.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether Formoterol is excreted in human milk; caution should be exercised when administered to a nursing mother. Consult a doctor.

WHO Essential Medicine
YesClinical Trials
Formoterol Fumarate has undergone extensive clinical trials demonstrating its efficacy and safety in the long-term management of asthma and COPD. Specific trials for a 200 mcg single-agent Formoterol inhalation capsule are not widely reported in standard literature, as this is an unusually high dose.
Lab Monitoring
- Regular monitoring of serum potassium levels, especially in patients with severe asthma or those receiving concomitant treatment with xanthine derivatives, steroids, or diuretics.
- Blood glucose levels, particularly in diabetic patients, due to potential for hyperglycemia.
Doctor Notes
- Emphasize the importance of proper inhaler technique to ensure optimal drug delivery.
- Stress that Brodil is a controller medicine and not a reliever for acute symptoms.
- Caution patients about the exceptionally high 200 mcg dose and to report any unusual or severe side effects immediately.
- Advise on the need for concomitant inhaled corticosteroid therapy for asthma patients.
- Monitor patients for cardiovascular effects, hypokalemia, and hyperglycemia, especially with this higher strength.
Patient Guidelines
- Use this medicine exactly as prescribed by your doctor and do not exceed the recommended dose.
- This capsule is for inhalation only using a specific dry powder inhaler device; do NOT swallow it.
- Brodil is a long-term maintenance medicine and should NOT be used for sudden breathing problems or acute asthma attacks. Always carry a fast-acting rescue inhaler for such situations.
- Ensure you understand the correct inhalation technique from your doctor or pharmacist.
- Continue using Brodil even if you feel better, unless advised otherwise by your doctor.
- Seek immediate medical attention if your breathing problems worsen or your rescue inhaler becomes less effective.
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for your next dose. In that case, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Brodil may cause side effects such as dizziness or tremor in some individuals. If you experience these symptoms, avoid driving or operating machinery until you are sure you can perform such activities safely.
Lifestyle Advice
- Avoid asthma/COPD triggers such as smoke, allergens, and pollutants.
- Quit smoking to improve lung health.
- Maintain good hydration and a healthy diet.
- Engage in regular physical activity as tolerated and advised by your doctor.
