Description
Overview of the medicine
Cephoral is a brand name for Cefixime, a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections including those of the respiratory tract, urinary tract, ear, nose, throat, and uncomplicated gonorrhea.
Uses & Indications
Dosage
Adults
Adults: 200 mg once daily or 100 mg twice daily. For severe infections or uncomplicated gonorrhea, 400 mg once daily.
Elderly
No special dosage adjustment needed unless renal impairment is present.
Renal_impairment
For creatinine clearance 21-60 ml/min, dose should be reduced to 75% of standard dose. For creatinine clearance <20 ml/min, dose should be reduced to 50% of standard dose.
How to Take
Can be taken with or without food. To minimize gastric upset, it is often recommended to take with food. Complete the full course of treatment as prescribed, even if symptoms improve.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby interfering with the transpeptidation step of peptidoglycan synthesis. This ultimately leads to the lysis and death of susceptible bacterial cells.
Pharmacokinetics
Onset
Peak plasma concentrations reached within 2-6 hours.
Excretion
Primarily excreted via the kidneys (glomerular filtration and tubular secretion); some biliary excretion also occurs.
Half life
Average plasma half-life is 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food slightly delays absorption but does not significantly affect the extent.
Metabolism
Not extensively metabolized. Approximately 50% is eliminated via urinary excretion as unchanged drug.
Side Effects
Contraindications
- •Known hypersensitivity to cefixime or other cephalosporins
- •History of anaphylactic reactions to penicillin
Drug Interactions
Probenecid
Increases cefixime plasma concentrations by inhibiting renal excretion.
Carbamazepine
Concomitant administration has resulted in increased carbamazepine levels.
Anticoagulants (e.g., Warfarin)
May increase prothrombin time; monitor INR.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Gastric lavage or induced emesis may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis.
Pregnancy & Lactation
Pregnancy Category B. Use with caution during pregnancy if clearly needed. Excreted in breast milk in small amounts; use with caution in breastfeeding mothers.
