Overview of the medicine
Ciprozid 250 mg Suspension is an antibiotic used to treat a variety of bacterial infections, including urinary tract, respiratory, skin, and gastrointestinal infections.
Uses & Indications
Dosage
Adults
Typically 250-750 mg twice daily, depending on the infection severity and type.
Elderly
Similar to adults, but dosage adjustment may be necessary in patients with impaired renal function.
Renal_impairment
Dosage adjustment required based on creatinine clearance; e.g., if CrCl 30-60 mL/min, 250-500 mg every 12 hours; if CrCl <30 mL/min, 250-500 mg every 18-24 hours.
How to Take
Shake well before use. Take orally with or without food. Avoid taking with dairy products or calcium-fortified juices alone. Do not chew or crush.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.
Pharmacokinetics
Onset
Within 1-2 hours
Excretion
Primarily renal (40-50% as unchanged drug), also fecal excretion.
Half life
Approximately 3-5 hours
Absorption
Rapid and well-absorbed after oral administration, bioavailability is approximately 70-80%.
Metabolism
Partially metabolized in the liver (10-20%) to less active metabolites.
Side Effects
Contraindications
- Hypersensitivity to ciprofloxacin or other quinolones
- Concomitant use with Tizanidine
- History of tendon disorders associated with quinolone use
- Children (except for specific severe infections)
Drug Interactions
Warfarin
May enhance anticoagulant effect, requiring INR monitoring.
Theophylline
Increased plasma levels of Theophylline, leading to increased risk of toxicity.
Corticosteroids
Increased risk of tendinopathy and tendon rupture.
Antacids/Dairy Products/Iron/Zinc
Reduced absorption of ciprofloxacin if taken concomitantly.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Gastric lavage, activated charcoal, maintain hydration. Symptomatic and supportive treatment. Hemodialysis or peritoneal dialysis is not effective in removing significant amounts of ciprofloxacin.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Ciprofloxacin is excreted in breast milk; advise against breastfeeding during treatment due to potential risks to the infant's joints and cartilage.

Doctor Notes
- Counsel patients on the importance of completing the full course and reporting any unusual symptoms, especially tendon pain, numbness, tingling, or weakness. Be aware of potential for QT prolongation in susceptible patients (e.g., those with existing cardiac conditions or on interacting medications). Use with caution in pediatric patients, weighing benefits against risks.

- Complete the full course of treatment even if symptoms improve to prevent antibiotic resistance.
- Avoid excessive sun exposure and use sun protection.
- Report any pain, swelling, or inflammation of tendons immediately.
- Drink plenty of fluids.
