Description
Overview of the medicine
Emixef 400 mg Capsule contains Cefixime, a broad-spectrum third-generation cephalosporin antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
400 mg once daily or 200 mg every 12 hours for 7-14 days depending on the infection.
Elderly
No dosage adjustment necessary unless severe renal impairment.
Renal_impairment
Reduce dose for creatinine clearance less than 60 mL/min. For CrCl <30 mL/min, 200 mg daily is recommended.
How to Take
May be taken with or without food. To minimize gastrointestinal upset, it is often recommended to take with food.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby preventing the cross-linking of peptidoglycan chains, which is essential for bacterial cell wall strength and rigidity. This leads to bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentration within 2-6 hours.
Excretion
Primarily excreted unchanged via urine (renal excretion) and bile (non-renal).
Half life
About 3-4 hours.
Absorption
Approximately 30-50% orally absorbed. Food slightly delays absorption but does not significantly affect the extent.
Metabolism
Not significantly metabolized; about 10% converted to inactive metabolites.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or other cephalosporins
- History of anaphylactic reactions to penicillins
Drug Interactions
Carbamazepine
Increased carbamazepine levels have been reported when co-administered with cefixime.
Oral contraceptives
May reduce efficacy of oral contraceptives (though clinical significance is generally low).
Anticoagulants (e.g., Warfarin)
May increase prothrombin time (INR) and risk of bleeding.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Symptomatic and supportive treatment is recommended.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Excreted in breast milk in small amounts; use with caution during lactation.
