Uses & Indications
Dosage
Adults
The usual adult dose is 400 mg once daily or 200 mg every 12 hours. For uncomplicated gonorrhea, a single 400 mg dose is recommended.
Elderly
No dosage adjustment is generally required for elderly patients unless there is significant renal impairment.
Renal_impairment
For patients with creatinine clearance less than 60 mL/min, the dose should be reduced (e.g., 200 mg once daily for CrCl < 60 mL/min; 100 mg once daily for CrCl < 30 mL/min).

Mechanism of Action
Cefixime exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby inhibiting the third and final stage of bacterial cell wall synthesis.

Peak plasma concentrations reached 2-6 hours after oral administration.
Excretion
Primarily excreted via the kidneys (renal excretion, approximately 50% unchanged in urine) and also via biliary excretion.

Approximately 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food may delay time to peak concentration but does not significantly alter total absorption.

Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of hypersensitivity to penicillin (due to potential cross-reactivity).
Drug Interactions
Warfarin
May increase prothrombin time (PT) and International Normalized Ratio (INR), increasing bleeding risk. Close monitoring of coagulation parameters is recommended.
Probenecid
May increase cefixime plasma concentrations and prolong its elimination by inhibiting renal excretion.
Carbamazepine
Concomitant administration may increase carbamazepine plasma concentrations. Monitor carbamazepine levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
