Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor (PPI) that decreases the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD), Zollinger-Ellison syndrome, and to heal erosive esophagitis.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. H. pylori: 40 mg once daily for 10 days (with antibiotics).
Elderly
No specific dosage adjustment is generally required, but caution is advised.
Renal_impairment
No dosage adjustment is necessary for patients with impaired renal function.
How to Take
Oral capsules should be swallowed whole, not chewed or crushed. Take at least one hour before a meal. Oral suspension can be mixed with water. IV injection is administered by a healthcare professional.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that works by irreversibly binding to and inhibiting the H+/K+-ATPase (proton pump) in the secretory canaliculi of gastric parietal cells, thereby suppressing gastric acid secretion.
Pharmacokinetics
Onset
Onset of acid suppression within 1 hour.
Excretion
Approximately 80% is excreted in the urine as metabolites, and the remainder in the feces.
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 64% after a single 40 mg dose and increases to 89% after repeated once-daily administration.
Metabolism
Extensively metabolized in the liver primarily by cytochrome P450 enzymes CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to esomeprazole or any component of the formulation, or to other substituted benzimidazoles.
- •Concomitant use with nelfinavir.
Drug Interactions
Warfarin
Increased INR and bleeding risk.
Clopidogrel
Reduced antiplatelet effect due to CYP2C19 inhibition.
Methotrexate
Increased methotrexate levels, potentially toxicity.
Nelfinavir, Atazanavir
Reduced antiviral efficacy due to decreased absorption.
Storage
Store below 30°C, in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms are generally mild and reversible. Treatment is symptomatic and supportive. Esomeprazole is extensively protein bound and not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but no adequate and well-controlled studies in pregnant women. Use during lactation only if potential benefit justifies potential risk.
