Overview of the medicine
Ciprofloxacin is a fluoroquinolone antibiotic used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
Typical oral dosage for most infections: 250-750 mg twice daily, depending on the severity and type of infection.
Elderly
Generally adult dosage, but renal function should be considered for dose adjustment.
Renal_impairment
Dosage adjustment is necessary for patients with impaired renal function (e.g., reduce dose for CrCl <30 mL/min).
How to Take
Take orally with or without food. Avoid taking with dairy products, calcium-fortified juices, or antacids within 2 hours before or 6 hours after ciprofloxacin.
Mechanism of Action
It works by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination.

Within 1-2 hours for therapeutic effect.
Excretion
Mainly excreted unchanged in urine (50-70%) and feces (15-30%).
Half life
Approximately 3-5 hours.
Absorption
Rapid and well-absorbed from the gastrointestinal tract after oral administration. Bioavailability is approximately 70%.
Metabolism
Limited hepatic metabolism, primarily through CYP1A2.
Side Effects
Contraindications
- Hypersensitivity to ciprofloxacin or other fluoroquinolones.
- Co-administration with tizanidine.
Drug Interactions
Warfarin
Increased anticoagulant effect, leading to increased bleeding risk.
Tizanidine
Significantly increased tizanidine plasma concentrations, leading to hypotension and somnolence.
Theophylline
Increased plasma levels of theophylline, leading to toxicity.
Antacids/Dairy products
Reduced absorption of ciprofloxacin.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. Gastric lavage and activated charcoal may be used. Hemodialysis and peritoneal dialysis remove only a small amount of ciprofloxacin.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Ciprofloxacin is excreted into breast milk; therefore, caution should be exercised when administered to a nursing woman.
