Description
Overview of the medicine
Lurasidone is an atypical antipsychotic medication used to treat schizophrenia in adults and adolescents (13-17 years of age), and depressive episodes associated with bipolar I disorder in adults.
Uses & Indications
Dosage
Adults
Schizophrenia: Initial dose 40 mg once daily, maximum 160 mg/day. Bipolar Depression: Initial dose 20 mg once daily, maximum 120 mg/day. Administer with food (at least 350 calories).
Elderly
No specific dosage adjustment recommended based solely on age, but caution should be exercised due to potential age-related organ impairments. Start with a lower dose and titrate slowly.
Renal_impairment
Moderate to severe renal impairment (CrCl <50 mL/min): Initial dose 20 mg/day; maximum dose 80 mg/day for schizophrenia, 60 mg/day for bipolar depression. No dose adjustment needed for mild impairment.
How to Take
Take orally once daily with food (at least 350 calories). Taking Lurasidone with food increases its absorption and effectiveness. Do not split, crush, or chew the tablet.
Mechanism of Action
Lurasidone is thought to act as an antagonist at dopamine D2 and serotonin 5-HT2A receptors, and as a partial agonist at serotonin 5-HT1A receptors. It also has antagonistic activity at alpha2C-adrenergic receptors.
Pharmacokinetics
Onset
Clinical improvement can be observed within days to weeks of initiating treatment, with full therapeutic effects taking several weeks.
Excretion
Eliminated primarily through feces (80%) and urine (9%).
Half life
Approximately 18 hours.
Absorption
Well absorbed after oral administration; bioavailability increases significantly with food (at least 350 calories). Peak plasma concentrations reached within 1-3 hours.
Metabolism
Extensively metabolized in the liver, primarily by the cytochrome P450 3A4 (CYP3A4) enzyme system.
Side Effects
Contraindications
- Known hypersensitivity to Lurasidone or any component of the formulation.
- Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin, ritonavir) and strong CYP3A4 inducers (e.g., rifampin, carbamazepine, phenytoin, St. John's wort).
Drug Interactions
Moderate CYP3A4 inhibitors (e.g., diltiazem)
Reduce lurasidone dose by half.
Moderate CYP3A4 inducers (e.g., bosentan, efavirenz)
Avoid concomitant use.
Strong CYP3A4 inducers (e.g., rifampin, carbamazepine)
Decrease lurasidone plasma concentrations; contraindicated.
Strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin)
Increase lurasidone plasma concentrations; contraindicated.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, sedation, tachycardia, and hypotension. Treatment should be symptomatic and supportive, including maintaining an open airway, oxygenation, and ventilation. Gastric lavage and activated charcoal may be considered. Closely monitor cardiovascular function.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Lurasidone is excreted in breast milk; therefore, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
