Uses & Indications
Dosage
Adults
Schizophrenia: Initial 40 mg once daily with food; target range 40-160 mg/day. Bipolar depression: Initial 20 mg once daily with food; target range 20-120 mg/day.
Elderly
Lower initial doses and slower titration may be considered. Max dose 80 mg/day (schizophrenia), 60 mg/day (bipolar depression) for patients with moderate-to-severe renal/hepatic impairment.
Renal_impairment
Moderate to severe (CrCl <50 mL/min): Max 80 mg/day (schizophrenia), max 60 mg/day (bipolar depression). No dose adjustment for mild impairment.
How to Take
Take orally once daily with food (at least 350 calories) to ensure proper absorption and reduce gastrointestinal upset.
Mechanism of Action
Lurasidone is an antagonist at dopamine D2 and serotonin 5-HT2A receptors, and a partial agonist at serotonin 5-HT1A receptors. It also has affinity for 5-HT7 and alpha2C-adrenergic receptors.

Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Management includes symptomatic and supportive treatment, including maintaining an open airway, oxygenation, ventilation, and cardiovascular monitoring. Gastric lavage and activated charcoal may be considered.

Pregnancy Category B. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as it is excreted in animal milk and potential for serious adverse reactions in breastfed infants.
