Overview of the medicine
Lurasidone is an atypical antipsychotic medication used to treat schizophrenia and depressive episodes associated with bipolar I disorder (bipolar depression) in adults and adolescents.
Uses & Indications
Dosage
Adults
Schizophrenia: Initial dose 40 mg once daily, taken with food. Recommended dose range is 40-160 mg once daily. Bipolar depression: Initial dose 20 mg once daily, taken with food. Recommended dose range is 20-120 mg once daily.
Elderly
No dose adjustment based on age alone. Caution is advised, and monitoring for renal/hepatic function is recommended. Maximum recommended dose for elderly patients with renal or hepatic impairment may be lower.
Renal_impairment
For moderate, severe, or ESRD (CrCl <50 mL/min), maximum recommended dose is 80 mg once daily. Dose adjustment may be necessary. Use with caution.
How to Take
Take Lurasidone orally once daily with food (at least 350 calories). Taking it without food significantly decreases its absorption.
Mechanism of Action
Lurasidone primarily acts as an antagonist at dopamine D2 and serotonin 5-HT2A receptors, and as a partial agonist at serotonin 5-HT1A receptors. It also has antagonistic activity at adrenergic alpha2C receptors and serotonin 5-HT7 receptors. This combined receptor activity is believed to contribute to its therapeutic effects on mood, thought, and behavior.

Clinical effects may be observed within a few weeks, with full therapeutic effect potentially taking longer.

Eliminated primarily via feces (approximately 80%) and urine (approximately 9%).

Side Effects
Contraindications

Strong CYP3A4 Inhibitors
Concomitant use is contraindicated due to significantly increased Lurasidone exposure. Examples: Ketoconazole, Clarithromycin.
Moderate CYP3A4 Inhibitors
Reduce Lurasidone dose by half. Examples: Diltiazem, Erythromycin.
Other CNS Depressants (e.g., alcohol, benzodiazepines)
May potentiate sedative effects. Use with caution.

Store at room temperature (20°C to 25°C), protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, sedation, tachycardia, and hypotension. There is no specific antidote for Lurasidone overdose. Management should involve supportive care, maintaining an adequate airway, oxygenation, and ventilation, and monitoring cardiac function and vital signs. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy: Category C. Lurasidone should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Neonates exposed to antipsychotic drugs during the third trimester of pregnancy are at risk for extrapyramidal and/or withdrawal symptoms. Lactation: It is not known whether Lurasidone is excreted in human milk. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
