Description
Overview of the medicine
Maxpro (Esomeprazole) is a proton pump inhibitor used to treat conditions like gastroesophageal reflux disease (GERD), erosive esophagitis, stomach ulcers, and Zollinger-Ellison syndrome by reducing stomach acid production.
Uses & Indications
Dosage
Adults
For GERD: 20-40 mg once daily for 4-8 weeks. For Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. For H. pylori eradication: 40 mg once daily for 10 days (in combination therapy).
Elderly
No dose adjustment generally required for elderly patients, but use with caution due to potential decreased hepatic function.
Renal_impairment
No dose adjustment needed for patients with renal impairment. Limited experience in severe renal impairment.
How to Take
Take orally, usually once daily, at least one hour before a meal. Do not crush or chew the capsule/tablet; swallow whole with water. If unable to swallow, capsules can be opened and contents mixed with a small amount of water or soft food and swallowed immediately.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells. By irreversibly blocking the final step in acid production, it effectively reduces both basal and stimulated gastric acid secretion.

Excretion
Primarily excreted in the urine (80%) and feces (20%).
Half life
Approximately 1.5 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentration reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 enzyme system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with Nelfinavir.
Drug Interactions
Digoxin
Increased systemic exposure to digoxin. Monitor digoxin levels.
Diazepam
Decreased clearance of diazepam (CYP2C19 substrate).
Warfarin
Increased INR and prothrombin time. Monitor closely.
Tacrolimus
Increased tacrolimus serum concentrations. Monitor tacrolimus levels and kidney function.
Clopidogrel
Reduced antiplatelet effect of clopidogrel. Avoid concomitant use.
Methotrexate
Increased and prolonged elevation of methotrexate and/or its metabolite serum levels. Consider temporary withdrawal of esomeprazole or dose reduction of methotrexate.
Nelfinavir, Atazanavir
Reduced plasma levels of antiretrovirals. Contraindicated with nelfinavir, avoid with atazanavir.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and may include drowsiness, tachycardia, nausea, diarrhea, headache. Management is symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Esomeprazole is likely excreted in human milk; caution should be exercised when administered to a nursing woman.
