Overview of the medicine
Metryl 200 mg suspension contains Metronidazole, an antibiotic and antiprotozoal medication used to treat a wide range of infections caused by anaerobic bacteria and certain parasites.
Uses & Indications
Dosage
Adults
Varies significantly by indication. For amoebiasis: 400-800 mg three times daily for 5-10 days. For giardiasis: 2000 mg once daily for 3 days or 250 mg three times daily for 5-7 days. For anaerobic infections: 400 mg three times daily for 7 days. Children's dosage for suspension is typically 15-50 mg/kg/day divided into 3 doses, depending on the infection.
Elderly
No specific dose reduction required, but use with caution due to potential decreased hepatic function.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe renal impairment or in patients undergoing dialysis, dose reduction (e.g., 50% of usual dose) may be necessary.
How to Take
Take orally, preferably with or after food, to minimize gastrointestinal upset. Shake the suspension well before use. Use a measuring spoon or cup for accurate dosing.
Mechanism of Action
Metronidazole enters bacterial and protozoal cells, where it is reduced by electron transport proteins in anaerobic conditions. The reduced form damages DNA and other macromolecules, leading to cell death.

Excretion

Side Effects
Contraindications
- •Hypersensitivity to metronidazole, other nitroimidazole derivatives, or any component of the formulation.
- •First trimester of pregnancy (relative contraindication, generally avoided unless absolutely necessary).
Drug Interactions
Alcohol
Disulfiram-like reaction (nausea, vomiting, flushing, tachycardia, headache, abdominal cramps) when consumed with or shortly after metronidazole. Avoid alcohol during and for at least 48 hours after treatment.
Lithium
Increases plasma lithium levels, potentially leading to lithium toxicity. Monitor lithium levels closely.
Cimetidine
May prolong metronidazole half-life, leading to increased plasma levels and potential toxicity.
Phenytoin, Phenobarbital
May decrease metronidazole plasma levels, reducing its efficacy.
Warfarin and other oral anticoagulants
Metronidazole can potentiate the anticoagulant effect, increasing the risk of bleeding. Close monitoring of INR and dose adjustment may be required.
Storage
Store below 30°C. Protect from light and moisture. Keep out of the reach and sight of children.
Overdose
Symptoms of overdose include nausea, vomiting, ataxia, seizures, and peripheral neuropathy. Management is symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Metronidazole crosses the placenta. Generally avoided during the first trimester unless the potential benefits outweigh the risks. Excreted into breast milk in significant amounts; caution advised. A decision to discontinue nursing or the drug should be made, considering the importance of the drug to the mother.
