Description
Overview of the medicine
Metronidazole 500 mg injection is an antimicrobial medication used to treat various bacterial and parasitic infections, particularly those caused by anaerobic bacteria and certain protozoa.
Uses & Indications
Dosage
Adults
For anaerobic infections: Initially 15 mg/kg IV infused over 30-60 minutes, then 7.5 mg/kg IV infused over 30-60 minutes every 6 hours for 7-10 days or longer depending on infection severity. Max 4g/day.
Elderly
No specific dosage adjustment needed, but caution advised due to potential for decreased hepatic function.
Renal_impairment
No dosage adjustment necessary for mild to moderate renal impairment. For severe renal impairment or hemodialysis, consider 50% dose reduction or administration after dialysis.
How to Take
Administer intravenously by slow infusion over 30-60 minutes. Do not administer as a rapid bolus.
Mechanism of Action
Metronidazole enters bacterial and protozoal cells, where its nitro group is reduced by electron transport proteins to reactive cytotoxic compounds. These compounds disrupt DNA structure and inhibit nucleic acid synthesis, leading to cell death.
Pharmacokinetics
Onset
Within hours of administration for clinical effect.
Excretion
Mainly excreted in the urine (60-80%) as parent drug and metabolites; some fecal excretion.
Half life
Approximately 8 hours (range 6-12 hours).
Absorption
Rapidly and almost completely absorbed after IV administration. Peak plasma concentrations are achieved within 1 hour.
Metabolism
Primarily metabolized in the liver by hydroxylation and oxidation, forming active metabolites.
