Description
Overview of the medicine
Metryl 500 mg tablet contains Metronidazole, an antibiotic effective against a wide range of bacterial and parasitic infections. It works by stopping the growth of certain bacteria and parasites.
Uses & Indications
Dosage
Adults
Dosage varies by indication. For amoebiasis: 500 mg three times daily for 5-10 days. For trichomoniasis: 250 mg three times daily for 7 days or 2 g single dose. For anaerobic infections: 500 mg two or three times daily.
Elderly
Use with caution; dose adjustment may be necessary in patients with significant hepatic impairment.
Renal_impairment
No dosage adjustment is usually necessary for mild to moderate renal impairment. For severe renal impairment or hemodialysis, the dose may need to be reduced or administered after dialysis.
How to Take
Take the tablet orally with or after food to minimize gastrointestinal upset. Swallow the tablet whole with water; do not crush or chew.
Mechanism of Action
Metronidazole is a nitroimidazole antimicrobial that enters the cell of susceptible organisms (anaerobic bacteria and protozoa). Once inside, it is reduced by electron transport proteins to reactive cytotoxic compounds that disrupt DNA synthesis and cause DNA damage, leading to cell death.
Pharmacokinetics
Onset
Therapeutic effects typically begin within hours, though full eradication of infection may take several days.
Excretion
Mainly excreted in urine (60-80%) as unchanged drug and metabolites, with a small amount excreted in feces.
Half life
The plasma elimination half-life is approximately 6-12 hours in adults.
Absorption
Metronidazole is well absorbed after oral administration, with peak plasma concentrations occurring within 1-3 hours.
Metabolism
Primarily metabolized in the liver by hydroxylation and glucuronidation, forming active and inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to metronidazole or other nitroimidazole derivatives
- •First trimester of pregnancy (relative contraindication)
Drug Interactions
Alcohol
Disulfiram-like reaction (nausea, vomiting, flushing, tachycardia)
Lithium
Increases plasma lithium concentrations, leading to toxicity
Cimetidine
May prolong metronidazole half-life by inhibiting hepatic metabolism
Phenytoin, Phenobarbital
May decrease metronidazole levels by inducing hepatic enzymes
Warfarin and other oral anticoagulants
Potentiates anticoagulant effect, increasing risk of bleeding
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, ataxia (lack of muscle coordination), disorientation, and seizures. Management is supportive and symptomatic, as there is no specific antidote.
Pregnancy & Lactation
Generally avoided in the first trimester of pregnancy. Use in the second and third trimesters only if clearly needed. Metronidazole is excreted into breast milk; breastfeeding should be temporarily discontinued during treatment and for 12-24 hours after the last dose.
