Description
Overview of the medicine
Metronidazole is an antibiotic and antiprotozoal medication used to treat a wide variety of infections, including amoebiasis, giardiasis, trichomoniasis, and bacterial vaginosis. It is also effective against anaerobic bacterial infections.
Uses & Indications
Dosage
Adults
Typical dose for amoebiasis: 400-800 mg three times daily for 5-10 days. For anaerobic infections: 400-500 mg three times daily. Doses vary significantly based on indication.
Elderly
Dosage adjustment may be necessary in elderly patients with hepatic impairment. Otherwise, adult dose generally applicable.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe impairment (CrCl < 10 mL/min), consider reducing dose or increasing dosing interval.
How to Take
Oral. Take tablets with or after food to minimize gastrointestinal upset. Swallow whole with water.
Mechanism of Action
Metronidazole is a nitroimidazole derivative. It enters bacterial and protozoal cells and is reduced to active metabolites, which then disrupt DNA structure and inhibit nucleic acid synthesis, leading to cell death.

Onset
Within 1-2 hours (oral).
Excretion
Mainly renal (60-80% of dose), small amount via feces.
Half life
Approximately 8 hours.
Absorption
Well absorbed orally, bioavailability ~80%.
Metabolism
Extensively metabolized in the liver by oxidation, primarily by CYP2C9 and CYP3A4, to hydroxy and acid metabolites.
Side Effects
Contraindications
- •Hypersensitivity to metronidazole or other nitroimidazole derivatives
- •First trimester of pregnancy (relative contraindication)
- •Alcohol consumption during treatment and for at least 3 days after (disulfiram-like reaction)
Drug Interactions
Alcohol
Disulfiram-like reaction (nausea, vomiting, flushing, tachycardia).
Lithium
Increased serum lithium levels, potential for toxicity.
Warfarin
Potentiates anticoagulant effect, increased risk of bleeding.
Cimetidine
May increase metronidazole levels due to reduced metabolism.
Phenytoin/Phenobarbital
Decreased metronidazole levels, reduced efficacy.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, ataxia, peripheral neuropathy, and seizures. Treatment is symptomatic and supportive; no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Generally avoided in the first trimester. Use with caution in later trimesters if clearly needed. Excreted into breast milk; breastfeeding should be temporarily discontinued or alternative medication considered.
