Description
Overview of the medicine
Montelukast is a leukotriene receptor antagonist used for the maintenance treatment of asthma and to relieve symptoms of seasonal allergic rhinitis. It works by blocking leukotrienes, which are natural substances that cause inflammation and tightening of airways.
Uses & Indications
Dosage
Adults
10 mg once daily in the evening.
Elderly
No dosage adjustment generally required.
Renal_impairment
No dosage adjustment required.
How to Take
Take orally once daily in the evening, with or without food.
Mechanism of Action
Montelukast selectively blocks the cysteinyl leukotriene 1 (CysLT1) receptor. Leukotrienes are potent inflammatory mediators released from various cells including mast cells and eosinophils, causing bronchoconstriction, mucus secretion, vascular permeability, and eosinophil recruitment. By blocking CysLT1 receptors, montelukast inhibits these effects.
Pharmacokinetics
Onset
Within 1 day for asthma symptoms, several days for full effect in allergic rhinitis.
Excretion
Primarily excreted in feces (approximately 86%), very little in urine.
Half life
Approximately 2.7 to 5.5 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentration reached in 2-4 hours for adults.
Metabolism
Extensively metabolized by CYP3A4, CYP2C9, and CYP2C8 in the liver.
Side Effects
Contraindications

Drug Interactions
Gemfibrozil
May increase montelukast plasma levels.
Phenobarbital
Decreases montelukast plasma levels.
Storage
Store below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
In cases of overdose, symptomatic and supportive measures should be employed. No specific antidote is known.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed and the potential benefit outweighs the potential risk to the fetus. It is unknown if montelukast is excreted in human milk, use with caution in nursing mothers.
