Description
Overview of the medicine
Nyclobate 0.05% Lotion contains Clobetasol Propionate, a highly potent corticosteroid, used for the short-term treatment of inflammatory and pruritic manifestations of moderate to severe corticosteroid-responsive dermatoses.
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin areas once or twice daily, for a maximum of 2 weeks. Do not use more than 50g per week. Discontinue when control is achieved.
Elderly
No specific dosage adjustment required, but use with caution due to potential for thinner skin and increased systemic absorption.
Renal_impairment
No specific dosage adjustment required, but systemic absorption should be monitored if used extensively.
How to Take
For external use only. Apply a thin layer to the affected skin and rub in gently. Wash hands after application. Avoid contact with eyes, mouth, and other mucous membranes. Do not use on the face, groin, or axillae, or for diaper dermatitis unless directed by a physician.
Mechanism of Action
Clobetasol Propionate is a topical corticosteroid that acts as an anti-inflammatory, anti-pruritic, and vasoconstrictive agent. It induces phospholipase A2 inhibitory proteins, lipocortins, which control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes by inhibiting the release of their common precursor arachidonic acid.
Pharmacokinetics
Onset
Within hours to days of application.
Excretion
Excreted primarily by the kidneys, and to a lesser extent, in the bile.
Half life
Plasma half-life is approximately 3-5 hours, but topical effects can last longer.
Absorption
Systemic absorption can occur, especially with prolonged use, application to large surface areas, or with occlusive dressings. Absorption is higher in areas with thin skin (e.g., face, groin).
Metabolism
Primarily metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to Clobetasol Propionate or any components of the formulation.
- Rosacea, acne vulgaris, perioral dermatitis.
- Pruritus without inflammation.
- Perianal and genital pruritus.
- Primary cutaneous viral infections (e.g., herpes simplex, chickenpox).
- Fungal or bacterial skin infections (unless concurrent appropriate anti-infective therapy is used).
- Tuberculosis of the skin.
Drug Interactions
No significant systemic drug interactions are expected with topical application.
However, if significant systemic absorption occurs, interactions similar to systemic corticosteroids (e.g., with CYP3A4 inhibitors like Ritonavir, Itraconazole) might be theoretically possible, leading to increased systemic exposure.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Topically applied Nyclobate 0.05% Lotion can be absorbed in sufficient amounts to produce systemic effects, including adrenal suppression. If HPA axis suppression is noted, withdraw the drug gradually, reduce the frequency of application, or substitute with a less potent corticosteroid.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether topical administration of corticosteroids could result in sufficient systemic absorption to produce detectable quantities in breast milk. Use with caution in nursing mothers. Consult a physician.
