Description
Overview of the medicine
Nyclobate 0.05% Ointment contains Clobetasol Propionate, a very potent corticosteroid used for short-term treatment of inflammatory skin conditions such as severe eczema, psoriasis, and lichen planus, which do not respond to less potent steroids.
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin area once or twice daily. Do not use for more than 2 consecutive weeks. Maximum weekly dose should not exceed 50g.
Elderly
Use with caution; consider lower frequency if skin atrophy is a concern.
Renal_impairment
No specific dosage adjustment required for topical application, but caution with extensive use due to potential systemic absorption.
How to Take
For external use only. Apply a thin layer gently to the affected skin area. Wash hands before and after application. Avoid contact with eyes, face, groin, and armpits unless specifically instructed by a doctor.
Mechanism of Action
Clobetasol Propionate is a corticosteroid that acts by inducing phospholipase A2 inhibitory proteins, thereby controlling the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes. It exhibits anti-inflammatory, antipruritic, and vasoconstrictive actions.
Pharmacokinetics
Onset
Onset of action is relatively rapid, typically within a few hours to days.
Excretion
Excreted mainly through the kidneys, with some biliary excretion.
Half life
The plasma half-life of absorbed clobetasol propionate is not well-defined for topical application but can be around 3-5 hours when systemically absorbed.
Absorption
Systemic absorption of topical clobetasol propionate is generally minimal, but can increase with prolonged use, application to large surface areas, occlusion, or compromised skin barrier. Absorption varies individually.
Metabolism
Primarily metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Clobetasol Propionate or any component of the ointment.
- Rosacea, acne vulgaris, perioral dermatitis.
- Pruritus without inflammation.
- Perianal and genital pruritus.
- Primary cutaneous viral infections (e.g., herpes simplex, chickenpox).
- Primary bacterial or fungal skin infections.
- Children under 1 year of age.
Drug Interactions
CYP3A4 inhibitors (e.g., Ritonavir, Itraconazole)
May increase systemic exposure to Clobetasol Propionate, leading to increased risk of systemic side effects. Use with caution.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Excessive or prolonged use of topical corticosteroids can suppress pituitary-adrenal function, leading to secondary adrenal insufficiency. Symptoms may include Cushing's syndrome, hyperglycemia, and glucosuria. Treatment involves gradual withdrawal of the steroid under medical supervision.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Clobetasol Propionate should not be used extensively on pregnant patients or for prolonged periods. Exercise caution when administering to a nursing mother. If used, avoid applying to the breast area.
