Overview of the medicine
Onepro (Esomeprazole) is a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD), erosive esophagitis, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. Erosive esophagitis: 40 mg once daily for 4-8 weeks. Zollinger-Ellison syndrome: Initial 40 mg twice daily, adjusted as needed.
Elderly
No dosage adjustment generally required, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment. For severe renal impairment, use with caution.
How to Take
Take orally at least one hour before a meal. Swallow whole, do not chew or crush.
Mechanism of Action
Esomeprazole works by irreversibly blocking the H+/K+-ATPase proton pump in the gastric parietal cells, thereby inhibiting the final step in acid production.
Pharmacokinetics
Onset
Acid suppression begins within 1 hour.
Excretion
Metabolites are primarily excreted in the urine (approx. 80%) and feces (approx. 20%).
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is about 64% for a 20 mg dose and 89% for a 40 mg dose, increasing with repeated dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole or any components of the formulation, or to other substituted benzimidazoles.
- Concomitant use with nelfinavir.
Drug Interactions
Warfarin
Increased INR and prothrombin time. Monitor closely.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel, especially in CYP2C19 poor metabolizers.
Methotrexate
Increased methotrexate levels. Consider temporary PPI withdrawal or lower dose.
Nelfinavir, Atazanavir
Decreased plasma levels of these antivirals due to increased gastric pH, leading to reduced efficacy. Concomitant use with nelfinavir is contraindicated.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include drowsiness, tachycardia, nausea, diarrhea. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Use in pregnancy only if clearly needed. Excreted in breast milk, use with caution during lactation.
