Onepro (Pantoprazole) is a proton pump inhibitor used to reduce stomach acid production, treating conditions like GERD, peptic ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
For GERD/Erosive Esophagitis: 40 mg once daily for up to 8 weeks. For Duodenal/Gastric Ulcers: 40 mg once daily for 2-4 weeks. For H. pylori eradication: 40 mg twice daily for 7-14 days with antibiotics. For Zollinger-Ellison Syndrome: Initial 40 mg twice daily, adjusted as needed.
Elderly
Generally, no dose adjustment is necessary for elderly patients.
Renal_impairment
No dose adjustment needed for patients with renal impairment.
Hepatic_impairment
For severe hepatic impairment, 20 mg once daily may be sufficient.
How to Take
Take the tablet whole, preferably in the morning before a meal. Do not chew, crush, or split the tablet.
Mechanism of Action
Pantoprazole irreversibly binds to the H+/K+-ATPase enzyme system (proton pump) in the gastric parietal cells, inhibiting the final step in acid production.

Onset
Within 2.5 hours
Excretion
Primarily via urine (approx. 80%) and feces (approx. 18%) as metabolites.
Half life
Approximately 1 hour (terminal half-life), but duration of acid suppression is much longer.
Absorption
Rapidly absorbed after oral administration, bioavailability about 77%. Peak plasma concentrations reached in 2-2.5 hours.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to pantoprazole or any component of the formulation.
- Hypersensitivity to other substituted benzimidazoles.
Drug Interactions
Warfarin
Monitor INR if co-administered.
Clopidogrel
Reduced antiplatelet effect of clopidogrel (avoid concomitant use if possible).
Methotrexate
May increase methotrexate levels (monitor carefully).
HIV Antivirals (e.g., Atazanavir)
Reduced absorption of antiretrovirals (avoid concomitant use).
Drugs dependent on gastric pH for absorption (e.g., Ketoconazole, Iron salts)
Reduced absorption due to increased gastric pH.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include lethargy, ataxia, tremor, abdominal pain. Treatment is symptomatic and supportive. Pantoprazole is not significantly removed by hemodialysis.

Pregnancy Category B. Use only if clearly needed. Excreted in breast milk, so caution advised during lactation.
