Description
Overview of the medicine
Renova 120 mg Syrup contains Paracetamol, which is used to relieve mild to moderate pain and to reduce fever. It is suitable for children and adults who prefer liquid formulations.
Uses & Indications
Dosage
Adults
10-20 mL (240-480 mg) every 4-6 hours as needed, not to exceed 80 mL (1920 mg) in 24 hours.
Elderly
Similar to adult dosage, but caution with hepatic/renal impairment. Consult physician.
Renal_impairment
Dosage interval may need to be extended (e.g., 6-8 hours) depending on creatinine clearance. Consult physician.
How to Take
Administer orally. Shake the bottle well before use. Use a measuring cup or spoon for accurate dosing.
Mechanism of Action
Paracetamol's exact mechanism is not fully understood, but it is believed to act primarily by inhibiting prostaglandin synthesis in the central nervous system (CNS), leading to its analgesic and antipyretic effects. It has minimal anti-inflammatory effects.
Pharmacokinetics
Onset
30-60 minutes for analgesic/antipyretic effect.
Excretion
Mainly excreted in urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
1-4 hours (typically 2-3 hours) in adults with normal hepatic function.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract; peak plasma concentrations occur within 30-60 minutes.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation pathways. A small portion is metabolized by cytochrome P450 enzymes to a toxic intermediate, N-acetyl-p-benzoquinone imine (NAPQI), which is detoxified by conjugation with glutathione.
Side Effects
Contraindications
- Known hypersensitivity to Paracetamol or any other component of the syrup
- Severe hepatic impairment or active liver disease
Drug Interactions
Alcohol
Chronic alcohol consumption may increase the risk of hepatotoxicity with paracetamol.
Warfarin
May enhance the anticoagulant effect of warfarin, increasing bleeding risk with prolonged use of high doses of paracetamol.
Chloramphenicol
May increase chloramphenicol levels.
Certain anticonvulsants (e.g., Phenytoin, Carbamazepine)
May increase paracetamol metabolism, potentially reducing its efficacy and increasing the risk of hepatotoxicity.
Storage
Store below 30°C in a cool, dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Overdose can cause severe, potentially fatal hepatic necrosis. Symptoms may include nausea, vomiting, abdominal pain, and diaphoresis, which may appear 24-48 hours after ingestion. Management involves gastric lavage, activated charcoal, and administration of N-acetylcysteine (NAC) as an antidote.
Pregnancy & Lactation
Paracetamol is generally considered safe for use during pregnancy and breastfeeding at recommended doses. However, it should be used with caution and under medical supervision.
