Description
Overview of the medicine
Rovantin (Atorvastatin) is a statin medication used to lower elevated cholesterol levels (LDL-C, total cholesterol, triglycerides) and increase HDL-C in patients with hyperlipidemia. It is also used to reduce the risk of cardiovascular events such as heart attack and stroke.
Uses & Indications
Dosage
Adults
Initial dose: 10 mg or 20 mg once daily. Max dose: 80 mg once daily.
Elderly
No dosage adjustment is generally required.
Renal_impairment
No dosage adjustment required.
How to Take
Oral administration, once daily, with or without food, at any time of day.
Mechanism of Action
Atorvastatin selectively inhibits HMG-CoA reductase, an enzyme that catalyzes the early rate-limiting step in cholesterol biosynthesis. This leads to a decrease in hepatic cholesterol synthesis and an increase in LDL receptors on liver cells, enhancing LDL-C uptake and catabolism.
Pharmacokinetics
Onset
Lipid-lowering effects seen within 2 weeks, maximal by 4 weeks.
Excretion
Primarily biliary excretion after hepatic and/or extrahepatic metabolism. Less than 2% is recovered in urine.
Half life
Parent drug half-life: ~14 hours. Active metabolites contribute to 20-30 hours of HMG-CoA reductase inhibitory activity.
Absorption
Rapidly absorbed. Peak plasma concentrations in 1-2 hours. Absolute bioavailability approx. 14%.
Metabolism
Extensive hepatic metabolism via CYP3A4 to active ortho- and parahydroxylated metabolites and various beta-oxidation products.

Contraindications
- Active liver disease or unexplained persistent elevations of serum transaminases
- Pregnancy and lactation
- Hypersensitivity to Atorvastatin or any component of the formulation
Drug Interactions

CYP3A4 Inhibitors (e.g., Itraconazole, Ketoconazole, Erythromycin, Clarithromycin, Ciclosporin, Protease inhibitors) can increase Atorvastatin levels, increasing risk of myopathy.

Fibrates (e.g., Gemfibrozil) and Niacin can increase the risk of myopathy/rhabdomyolysis when co-administered with Atorvastatin.

Colchicine: Cases of myopathy, including rhabdomyolysis, have been reported with coadministration of colchicine and atorvastatin.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
There is no specific treatment for atorvastatin overdose. In the event of an overdose, the patient should be treated symptomatically, and supportive measures instituted as required. Liver function tests and CK levels should be monitored.
Pregnancy & Lactation
Atorvastatin is contraindicated during pregnancy and lactation due to the potential for harm to the fetus or breastfed infant. Women of childbearing potential should use effective contraception.
