Description
Overview of the medicine
Roxim (Cefuroxime) is a broad-spectrum, second-generation cephalosporin antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
250 mg to 500 mg orally twice daily, depending on the infection severity and type.
Elderly
Generally, adult dosage applies unless significant renal impairment is present. Dosage adjustment may be required based on renal function.
Renal_impairment
Dosage should be reduced in patients with significant renal impairment (CrCl < 30 mL/min). Consult prescribing information for specific guidelines.
How to Take
Oral tablets/suspension should be taken with food to optimize absorption and reduce potential gastrointestinal discomfort. Injectable forms are administered intravenously or intramuscularly.
Mechanism of Action
Cefuroxime is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations reached approximately 2-3 hours after oral administration.
Excretion
Primarily via the kidneys by glomerular filtration and tubular secretion.
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Well absorbed orally, enhanced by food. Bioavailability of oral tablets is about 37% to 52%.
Metabolism
Not extensively metabolized in the body. The drug is largely excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to cefuroxime, other cephalosporins, or to any of the excipients.
- History of severe hypersensitivity reaction to any other type of beta-lactam antibacterial agents (e.g., penicillins).
Drug Interactions
Probenecid
Increases cefuroxime plasma concentrations and prolongs its half-life by reducing renal tubular secretion.
Loop diuretics
May enhance nephrotoxic potential.
Aminoglycosides
Increased risk of nephrotoxicity.
Antacids/H2 blockers
Can reduce bioavailability of oral cefuroxime (acid suppression reduces absorption).
Oral anticoagulants (e.g., warfarin)
Prothrombin time may be prolonged; monitor INR.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally consistent with the side effect profile and include nausea, vomiting, and diarrhea. Convulsions can occur. Management is supportive; hemodialysis can aid in removal from the blood.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of impaired fertility or harm to the fetus. Use during pregnancy only if clearly needed. Excreted in human milk in small quantities; caution should be exercised when administered to a nursing woman.

YesClinical Trials
Extensive clinical trials have established the efficacy and safety of cefuroxime for its approved indications. Ongoing post-marketing surveillance continues to monitor its profile.
Lab Monitoring
- Regular monitoring of renal function is advised in patients receiving high doses or prolonged therapy, especially in those with pre-existing renal impairment. Liver function tests may be monitored if clinically indicated.
- Monitor INR/PT in patients on oral anticoagulants.
Doctor Notes
- Prescribe based on sensitivity testing results if available.
- Counsel patients on completing the full course and potential side effects.
- Monitor for signs of superinfection.
Patient Guidelines
- Complete the full course of antibiotics, even if symptoms improve, to prevent resistance.
- Take with food to improve absorption and reduce stomach upset.
- Inform your doctor about any allergies, especially to penicillin or other antibiotics.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Roxim may cause dizziness. Patients should be cautioned about operating machinery or driving until they are reasonably certain that cefuroxime does not adversely affect their ability to engage in such activities.
Lifestyle Advice
- Maintain adequate hydration.
- Avoid self-medication with antibiotics.
- Consult a doctor if symptoms persist or worsen.
