Description
Overview of the medicine
Cefuroxime is a second-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. It is effective against both Gram-positive and Gram-negative bacteria.
Uses & Indications
Dosage
Adults
Typically 750 mg to 1.5 gm IV/IM every 8 hours, depending on the severity of infection. For surgical prophylaxis, 1.5 gm IV at induction of anaesthesia.
Elderly
Dosage adjustment may be necessary in elderly patients with reduced renal function. Monitor renal function.
Renal_impairment
Dosage reduction is required for patients with creatinine clearance < 30 mL/min. (e.g., 750 mg every 12-24 hours).
How to Take
For intravenous administration, reconstitute the powder with sterile water for injection and administer slowly over 3-5 minutes or as an infusion over 30 minutes. For intramuscular administration, reconstitute with sterile water for injection and inject deeply into a large muscle mass.
Mechanism of Action
Cefuroxime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), which are enzymes involved in the final stages of peptidoglycan synthesis, thereby preventing the formation of a stable cell wall and leading to bacterial lysis.
Pharmacokinetics
Onset
Peak plasma concentrations reached within 30-60 minutes after IM/IV administration.
Excretion
Excreted primarily by the kidneys via glomerular filtration and tubular secretion.
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Rapidly absorbed after intramuscular injection. Bioavailability is high.
Metabolism
Not significantly metabolized; primarily excreted as unchanged drug.
Side Effects
Contraindications
- Hypersensitivity to cefuroxime or other cephalosporin antibiotics
- History of severe hypersensitivity reaction to any other type of beta-lactam antibacterial agent (e.g., penicillins, carbapenems)
Drug Interactions
Probenecid
Increases plasma concentration and prolongs the half-life of cefuroxime.
Aminoglycosides
Increased risk of nephrotoxicity.
Oral Contraceptives
May reduce the efficacy of oral contraceptives; advise additional contraception.
Loop Diuretics (e.g., Furosemide)
May increase nephrotoxicity of cephalosporins.
Oral Anticoagulants (e.g., Warfarin)
May enhance anticoagulant effect; monitor INR.
Storage
Store below 30°C. Protect from light and moisture. Reconstituted solution should be used immediately or within 24 hours if refrigerated.
Overdose
Symptoms of overdose include convulsions, encephalopathy, and neuromuscular excitability. Management is supportive, including hemodialysis for severe cases.
Pregnancy & Lactation
Pregnancy category B. Use during pregnancy only if clearly needed. Excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but monitor infant for adverse effects.
