Cefuroxime is a second-generation cephalosporin antibiotic used to treat a wide range of bacterial infections.

Uses & Indications

Upper and lower respiratory tract infections (e.g., bronchitis, pneumonia, sinusitis)

Urinary tract infections (e.g., pyelonephritis, cystitis)

Skin and soft tissue infections (e.g., furunculosis, impetigo)

Otitis media

Gonorrhea (uncomplicated)

Dosage

Adults

For most infections, 250 mg twice daily. For more severe infections or lower respiratory tract infections, 500 mg twice daily. For uncomplicated gonorrhea, a single 1000 mg dose.

Elderly

No specific dose adjustment is required unless there is significant renal impairment. Monitor renal function.

Renal_impairment

Dose adjustment needed based on creatinine clearance. For CrCl 10-29 mL/min, 250 mg once daily. For CrCl <10 mL/min, 250 mg every 48 hours.

Take orally with food for optimum absorption.

Mechanism of Action

Cefuroxime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death.

Pharmacokinetics

Onset

Peak plasma concentrations reached approximately 2-3 hours after oral administration.

Excretion

Excreted unchanged primarily by the kidneys via glomerular filtration and tubular secretion.

Half life

Approximately 1.2 to 1.6 hours.

Absorption

Well absorbed from the gastrointestinal tract after oral administration (as axetil ester), with enhanced absorption when taken with food.

Metabolism

Cefuroxime axetil is rapidly hydrolyzed by esterase enzymes in the intestinal mucosa and blood to release cefuroxime. Cefuroxime itself is not significantly metabolized.

Side Effects

Contraindications

Known hypersensitivity to cefuroxime, any other cephalosporin antibiotics, or any component of the formulation.
History of severe hypersensitivity reactions (e.g., anaphylaxis) to any other type of beta-lactam antibacterial agents (e.g., penicillins).

Drug Interactions

Antacids

Drugs that reduce gastric acidity may reduce the bioavailability of cefuroxime axetil.

Probenecid

Concomitant administration with probenecid increases the area under the plasma concentration-time curve (AUC) of cefuroxime.

Oral Contraceptives

May affect gut flora, leading to lower estrogen reabsorption and reduced efficacy of combined oral contraceptives.

Storage

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Overdose

Overdose can lead to neurological sequelae including convulsions. Management is supportive and symptomatic. Hemodialysis may be used to reduce cefuroxime serum levels.

Pregnancy & Lactation

Pregnancy Category B. Studies in animals have shown no evidence of impaired fertility or harm to the fetus. Cefuroxime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.
