Description
Overview of the medicine
Sultolin (Salbutamol) is a short-acting beta-2 adrenergic agonist used for the relief of bronchospasm in conditions like asthma and chronic obstructive pulmonary disease (COPD). It works by relaxing the smooth muscles of the airways, making breathing easier.
Uses & Indications
Dosage
Adults
For acute bronchospasm: 100 or 200 mcg (1 or 2 puffs) as a single dose. For maintenance: 200 mcg (2 puffs) three or four times daily. Maximum 8 puffs in 24 hours.
Elderly
Similar to adult dosage, but caution should be exercised and lower initial doses may be considered.
Renal_impairment
No specific dose adjustment is generally required, but caution is advised in severe impairment.
How to Take
For inhalers, shake the device well before use. Exhale fully, then inhale slowly and deeply through the mouth while pressing down on the canister. Hold breath for 5-10 seconds, then exhale slowly. Wait 1 minute before taking a second puff if prescribed. Oral forms are taken with water.
Mechanism of Action
Salbutamol selectively stimulates beta-2 adrenergic receptors, primarily located in the bronchial smooth muscles. This stimulation leads to the activation of adenyl cyclase, increasing intracellular cyclic AMP (cAMP) levels. Elevated cAMP levels result in relaxation of bronchial smooth muscles, leading to bronchodilation and improved airflow.
Pharmacokinetics
Onset
5-15 minutes after inhalation.
Excretion
Primarily excreted in urine as unchanged drug and its metabolite.
Half life
Approximately 2.7 to 5 hours.
Absorption
Rapidly absorbed after oral inhalation, with approximately 10-20% reaching the lungs. The remainder is swallowed and absorbed from the gastrointestinal tract.
Metabolism
Metabolized in the liver to an inactive sulfate conjugate.
Side Effects
Contraindications
- Hypersensitivity to salbutamol or any component of the formulation.
- Severe milk protein allergy (for some formulations).
Drug Interactions
Digoxin
May decrease digoxin levels.
Diuretics
May potentiate hypokalemia, especially with high doses.
MAOIs/TCAs
Increased risk of cardiovascular effects.
Beta-blockers
May antagonize the bronchodilatory effect of salbutamol.
Storage
Store below 30°C. Protect from frost and direct sunlight. Keep out of reach of children. Do not puncture or incinerate the canister, even when empty.
Overdose
Symptoms of overdose include tachycardia, palpitations, tremor, headache, and hypokalemia. Management involves symptomatic and supportive treatment, including monitoring of serum potassium levels and cardiac function.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Consult a doctor. Caution should be exercised when administered to a nursing mother. It is unknown if salbutamol is excreted in human milk.

WHO Essential Medicine
YesClinical Trials
Salbutamol has undergone extensive clinical trials demonstrating its efficacy and safety for bronchodilation in asthma and COPD. Ongoing research continues to explore its long-term effects and optimal usage in various patient populations.
Lab Monitoring
- Serum potassium levels (especially in patients receiving high doses, co-administered diuretics, or with severe asthma).
- Blood glucose (in diabetic patients).
Doctor Notes
- Emphasize proper inhaler technique to patients.
- Educate patients on distinguishing between rescue inhaler (salbutamol) and controller medications.
- Monitor for signs of overuse or worsening asthma control, which may indicate a need for step-up in controller therapy.
Patient Guidelines
- Understand proper inhaler technique for effective delivery of the medicine.
- Do not exceed the prescribed dose or frequency.
- Seek medical attention if symptoms worsen or if the usual dose does not provide relief.
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for your next dose. In that case, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
Driving Precautions
Salbutamol generally does not affect the ability to drive or operate machinery. However, if side effects like tremor, dizziness, or palpitations occur, caution should be exercised.
Lifestyle Advice
- Identify and avoid asthma triggers.
- Maintain a healthy lifestyle, including regular exercise (if appropriate).
- Avoid smoking and exposure to secondhand smoke.
