Tdcef 400 mg Capsule is an oral third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
400 mg once daily or 200 mg every 12 hours for 7-14 days depending on the infection. For uncomplicated gonorrhea, a single 400 mg dose.
Elderly
No specific dose adjustment required unless significant renal impairment is present.
Renal_impairment
Creatinine clearance < 60 mL/min: dosage adjustment may be needed; <30 mL/min: 200 mg once daily.
How to Take
Tdcef Capsule should be swallowed whole with water, with or without food. Complete the full course of treatment as prescribed.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis, leading to bacterial cell lysis and death. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall.
Pharmacokinetics
Onset
Plasma peak concentrations occur 2-6 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine (about 50%) and bile.
Half life
About 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food slightly delays time to peak concentration but does not significantly affect overall absorption.
Metabolism
Minimal hepatic metabolism; no active metabolites identified.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of serious hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Warfarin
May increase anticoagulant effect, requiring closer monitoring of INR.
Nifedipine
Concurrent use may increase cefixime peak plasma concentration and AUC.
Carbamazepine
Increased carbamazepine plasma levels have been reported with concurrent cefixime use.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. There is no specific antidote. Hemodialysis and peritoneal dialysis are generally not effective in removing cefixime from the body. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but adequate and well-controlled studies in pregnant women are lacking. Use only if clearly needed. Excreted in breast milk in small amounts; use with caution.
