Description
Overview of the medicine
Cefixime is an oral third-generation cephalosporin antibiotic effective against a wide range of Gram-positive and Gram-negative bacteria. It is commonly used to treat various bacterial infections in children.
Uses & Indications
Dosage
Adults
Not typically recommended for adults in this strength/form. Adult dosages of Cefixime are usually 200-400 mg tablets, once or twice daily.
Elderly
Not applicable for this strength/form, as it is primarily for pediatric use. Dosage adjustment may be required for elderly patients with severe renal impairment.
Renal_impairment
For children with CrCl < 30 ml/min, dosage adjustment is required. The dose should be reduced by 50% or the dosing interval extended.
How to Take
Oral administration. Shake the bottle well before each use to ensure uniform suspension. Can be taken with or without food. Use the provided measuring device for accurate dosing.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the final transpeptidation step of peptidoglycan synthesis, leading to bacterial cell lysis and death.

Onset
Onset of action depends on the infection type and severity; generally, clinical improvement seen within 24-48 hours.
Excretion
Primarily excreted unchanged in urine via glomerular filtration and tubular secretion (about 50%), and some through biliary excretion.
Half life
Approximately 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food may delay absorption but does not significantly affect the extent.
Metabolism
Not extensively metabolized in the body. Primary metabolic pathway involves desacetylation.

Drug Interactions
Probenecid
May increase Cefixime plasma concentrations by inhibiting renal excretion.
Carbamazepine
Concomitant use may increase carbamazepine plasma concentrations.
Warfarin and other anticoagulants
May increase prothrombin time (PT) and International Normalized Ratio (INR), leading to an increased risk of bleeding. Close monitoring is recommended.
Storage
Store the dry powder below 30°C, away from moisture and direct sunlight. Keep the reconstituted suspension in a refrigerator (2-8°C) and protect from light.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Supportive care and symptomatic treatment are recommended.
