Overview of the medicine
Turbocef 1.5 gm Injection is a broad-spectrum antibiotic combination of Cefoperazone (a third-generation cephalosporin) and Sulbactam (a beta-lactamase inhibitor) used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
Usual adult dose is 2-4 gm daily in equally divided doses every 12 hours. For severe infections, up to 8 gm daily.
Elderly
No specific dose adjustment is required unless there is severe renal or hepatic impairment. Monitor liver/kidney function.
Renal_impairment
In severe renal impairment (creatinine clearance <18 mL/min), maximum sulbactam dose is 1 gm daily. Cefoperazone dose adjustment may not be necessary due to dual excretion.
How to Take
Administered by slow intravenous infusion over 15-60 minutes or by deep intramuscular injection. Reconstitute powder with sterile water for injection or compatible diluent.
Mechanism of Action
Cefoperazone inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). Sulbactam inhibits common bacterial beta-lactamases, protecting Cefoperazone from degradation and expanding its spectrum of activity against resistant bacteria.
Pharmacokinetics
Onset
Rapid, typically within minutes of IV administration.
Excretion
Primarily excreted unchanged via bile and urine (Cefoperazone), and primarily via urine (Sulbactam).
Half life
Cefoperazone: approximately 1.7 hours; Sulbactam: approximately 1 hour.
Absorption
Rapidly and completely absorbed after IM/IV administration. Peak plasma levels achieved within 30 minutes (IV) to 1-2 hours (IM).
Metabolism
Cefoperazone is minimally metabolized; Sulbactam is not significantly metabolized.
Side Effects
Contraindications
- Hypersensitivity to cefoperazone, sulbactam, or any cephalosporin antibiotic
- History of severe hypersensitivity reaction to any other beta-lactam antibiotic
Drug Interactions
Alcohol
Disulfiram-like reaction (flushing, sweating, headache, tachycardia) may occur due to N-methylthiotetrazole side chain of cefoperazone.
Aminoglycosides
Potential for additive nephrotoxicity and ototoxicity if used concurrently.
Anticoagulants (Warfarin)
May enhance the anticoagulant effect by interfering with vitamin K synthesis; monitor INR.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children. Reconstituted solution should be used immediately or stored as per specific instructions for a limited period.
Overdose
Symptoms may include neurological effects like seizures. Treatment is supportive; hemodialysis may be useful in patients with impaired renal function.
