Description
Overview of the medicine
Cefuroxime is a second-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Most infections: 250 mg twice daily. Severe infections or lower respiratory tract infections: 500 mg twice daily. Uncomplicated UTIs: 125-250 mg twice daily. Uncomplicated gonorrhoea: 1000 mg as a single dose. Early Lyme disease: 500 mg twice daily for 20 days.
Elderly
Dosage should be as for adults, but renal function should be considered and dosage adjusted if necessary.
Renal_impairment
Creatinine clearance 10-29 mL/min: 250 mg once daily. Creatinine clearance <10 mL/min: 125 mg once daily. Haemodialysis: An additional 125 mg dose should be given at the end of each dialysis.
How to Take
Oral. For optimal absorption, it should be taken with food. Do not crush the tablet.
Mechanism of Action
Cefuroxime is a bactericidal agent that acts by inhibiting bacterial cell wall synthesis. It has a high affinity for penicillin-binding proteins (PBPs), leading to the disruption of peptidoglycan synthesis.
Pharmacokinetics
Onset
Peak plasma concentrations are reached approximately 2-3 hours after oral administration with food.
Excretion
Excreted unchanged, primarily by the kidneys (approximately 50% by glomerular filtration and 50% by tubular secretion), within 24 hours.
Half life
Approximately 1 to 1.5 hours.
Absorption
Cefuroxime axetil is absorbed from the gastrointestinal tract and rapidly hydrolyzed by esterase enzymes in the intestinal mucosa and blood to release cefuroxime into the circulation. Absorption is enhanced by food. Bioavailability is approximately 37% for tablets in fasting state, increasing to 52% with food.
Metabolism
Cefuroxime is not metabolized.
Side Effects
Contraindications
- Known hypersensitivity to cefuroxime or to any of the excipients.
- Known hypersensitivity to any cephalosporin antibiotic.
- History of severe hypersensitivity reaction to any other type of beta-lactam antibacterial agent (e.g., penicillins, monobactams and carbapenems).
Drug Interactions
Probenecid
Concomitant administration with probenecid increases the area under the serum concentration versus time curve (AUC) of cefuroxime by 50%.
Oral Contraceptives
May reduce the efficacy of oral contraceptives. Patients should use additional non-hormonal contraception.
Antacids (containing aluminium/magnesium hydroxide)
May decrease the bioavailability of cefuroxime. Separate administration by several hours.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdosage of cefuroxime can lead to neurological sequelae including encephalopathy, convulsions, and coma. Management includes supportive care. Cefuroxime serum levels can be reduced by hemodialysis and peritoneal dialysis.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of impaired fertility or harm to the fetus. Cefuroxime is excreted in human milk in small quantities. Use during pregnancy and lactation should be undertaken only after a careful risk/benefit assessment by a physician.
