Midazolam is a short-acting benzodiazepine used for sedation, anxiety reduction, and induction of anesthesia. It exhibits anxiolytic, sedative, hypnotic, anticonvulsant, and muscle relaxant properties, making it suitable for various medical procedures.
Uses & Indications
Dosage
Adults
Premedication: 0.07-0.08 mg/kg IM, 30-60 min before surgery. Conscious Sedation: Initial 0.02-0.035 mg/kg IV, slowly over 2-3 min, may titrate. Induction of Anesthesia: 0.1-0.3 mg/kg IV. ICU Sedation: Loading dose 0.01-0.05 mg/kg IV, followed by continuous infusion 0.02-0.1 mg/kg/hr.
Elderly
Lower doses are required due to increased sensitivity and slower elimination. Reduce initial dose by 30-50%. Careful titration is essential.
Renal_impairment
Dose adjustment may be necessary. Accumulation of active metabolites possible; monitor carefully. Reduce dose in severe impairment.
How to Take
Administered intravenously (IV) or intramuscularly (IM) by a healthcare professional. IV administration should be slow, over at least 2 minutes. Continuous IV infusion requires a controlled infusion device.
Mechanism of Action
Midazolam enhances the effect of gamma-aminobutyric acid (GABA), the major inhibitory neurotransmitter in the central nervous system, by binding to benzodiazepine receptors on the GABA-A receptor complex. This action leads to increased chloride ion influx, neuronal hyperpolarization, and decreased neuronal excitability, resulting in its sedative, anxiolytic, and anticonvulsant effects.

Side Effects
Contraindications

Drug Interactions
Opioids, Alcohol, Other CNS Depressants
Potentiate sedative and respiratory depressant effects. Avoid concomitant use or reduce doses significantly.
CYP3A4 Inducers (e.g., Rifampin, Phenytoin)
Decrease Midazolam plasma concentrations, reducing its effectiveness.
CYP3A4 Inhibitors (e.g., Ketoconazole, Erythromycin, Grapefruit juice)
Increase Midazolam plasma concentrations, prolonging its effects. Dose reduction needed.
Storage
Store below 30°C (86°F). Do not freeze. Protect from light. Keep out of reach of children.
Overdose
Symptoms include somnolence, confusion, impaired coordination, diminished reflexes, coma, and respiratory depression. Management involves supportive care, maintaining airway, and administering flumazenil (benzodiazepine antagonist) if severe respiratory depression or sedation occurs.
Pregnancy & Lactation
Pregnancy: Category D. Can cause fetal harm. Increased risk of congenital malformations in the first trimester. If used late in pregnancy, can cause neonatal flaccidity, respiratory depression, feeding difficulties, and withdrawal symptoms. Lactation: Excreted in breast milk. Use with caution. Advise mothers to avoid breastfeeding for at least 4-6 hours after a single dose.
