Overview of the medicine
Windel 2 mg Tablet contains Glimepiride, an oral anti-diabetic medicine used to lower blood glucose levels in patients with type 2 diabetes mellitus. It is used as an adjunct to diet and exercise.
Uses & Indications
Dosage
Adults
Initial dose 1-2 mg once daily with breakfast or first main meal. Max 8 mg once daily.
Elderly
Start with 1 mg daily and adjust carefully, monitoring glucose levels closely.
Renal_impairment
Initial dose 1 mg once daily, careful titration required due to risk of hypoglycemia.
How to Take
Oral administration. Take once daily with breakfast or the first main meal. Swallow whole with water, do not crush or chew.
Mechanism of Action
Glimepiride primarily lowers blood glucose by stimulating the release of insulin from the pancreatic beta cells. It also increases the sensitivity of peripheral tissues to insulin.

Onset of action typically occurs within 1 hour.
Excretion
Mainly excreted by the kidneys (approx. 60%) and feces (approx. 40%) as metabolites.
Half life
Approximately 5-9 hours.
Absorption
Rapidly and completely absorbed from the GI tract after oral administration. Peak plasma concentrations are reached in 2-3 hours.
Metabolism
Extensively metabolized in the liver by CYP2C9 to inactive metabolites.

Drug Interactions
Beta-blockers
May mask symptoms of hypoglycemia.
Fluconazole, NSAIDs, Warfarin
May potentiate the hypoglycemic effect.
Corticosteroids, Thiazide diuretics
May reduce the hypoglycemic effect.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include hypoglycemia (sweating, tremor, hunger, confusion). Management involves administration of glucose (oral or IV), close monitoring, and supportive care.
Pregnancy & Lactation
Contraindicated during pregnancy and lactation due to potential harm to the fetus/infant. Insulin is generally preferred for blood glucose control during pregnancy.
