Description
Overview of the medicine
Windel 200 mcg Inhalation Capsule contains Budesonide, an inhaled corticosteroid used for the maintenance treatment of asthma and chronic obstructive pulmonary disease (COPD). It helps to reduce inflammation in the airways.
Uses & Indications
Dosage
Adults
Asthma: Usually 200 mcg twice daily. May be increased up to 400 mcg twice daily in severe cases. COPD: 200-400 mcg twice daily.
Elderly
Similar dosage as adults, monitor for potential systemic effects.
Renal_impairment
No specific dosage adjustment required, but use with caution in severe impairment.
How to Take
For oral inhalation only, using a dry powder inhaler device (e.g., Respules, Turbuhaler). Do not swallow the capsule. Rinse mouth thoroughly with water after each use to prevent oral candidiasis.
Mechanism of Action
Budesonide works by decreasing inflammation and swelling in the airways. It suppresses the migration of inflammatory cells and reverses increased capillary permeability and dilation. This leads to a reduction in bronchoconstriction and hyperresponsiveness.
Pharmacokinetics
Onset
Therapeutic effects may be observed within a few days, but full benefits usually take 1-2 weeks of regular use.
Excretion
Metabolites are primarily excreted via urine (approximately 60%) and feces.
Half life
Approximately 2 to 3 hours in adults.
Absorption
Rapidly absorbed from the lung; systemic bioavailability is low (approximately 10-15%) due to extensive first-pass metabolism in the liver.
Metabolism
Extensively metabolized in the liver by cytochrome P450 3A4 (CYP3A4) to inactive metabolites.
Side Effects
Contraindications
- •Acute bronchospasm or status asthmaticus (Budesonide is not a rescue inhaler).
- •Hypersensitivity to Budesonide or any component of the formulation.
Drug Interactions
Oral contraceptives
May increase plasma concentrations of budesonide.
Ketoconazole, Itraconazole, Ritonavir, Atazanavir
Strong CYP3A4 inhibitors can significantly increase systemic exposure to budesonide, leading to potential systemic corticosteroid effects. Concomitant use should be avoided or closely monitored.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Acute overdose with Budesonide inhalation capsules is unlikely to cause clinically significant problems. Chronic overuse (several months to years) may lead to systemic corticosteroid effects such as hypercorticism and adrenal suppression. Treatment is supportive; gradual dose reduction should be considered.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown a risk to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Budesonide is excreted in human milk; caution should be exercised when administered to a nursing woman.
