Description
Overview of the medicine
Zedoxim (Cefixime) is a third-generation cephalosporin antibiotic used to treat a variety of bacterial infections, including those of the urinary tract, ear, throat, and respiratory system. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Typically, 200 mg once daily or 100 mg twice daily. For severe infections, 400 mg once daily or 200 mg twice daily. Duration usually 7-14 days depending on infection.
Elderly
No dose adjustment generally required unless there is significant renal impairment.
Renal_impairment
For creatinine clearance 21-60 ml/min, dose should be reduced by 25%. For creatinine clearance < 20 ml/min, dose should be reduced by 50% or frequency altered.
How to Take
Take orally with a glass of water, with or without food. Complete the full course of treatment as prescribed by the doctor, even if symptoms improve.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the final transpeptidation step of peptidoglycan synthesis. This leads to the disruption of the bacterial cell wall and eventual cell death.
Pharmacokinetics
Onset
Approximately 2-6 hours to achieve peak plasma concentration.
Excretion
Primarily excreted by the kidneys (approximately 50% of the absorbed dose) as unchanged drug in the urine; a small percentage is excreted via bile.
Half life
Average plasma half-life is 3-4 hours, but can be prolonged in renal impairment.
Absorption
Approximately 40-50% of an oral dose is absorbed from the gastrointestinal tract. Food delays the time to maximal absorption but does not significantly alter the extent of absorption.
Metabolism
Cefixime is not significantly metabolized in the liver.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of serious hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Carbamazepine
May increase carbamazepine levels when co-administered. Therapeutic drug monitoring may be necessary.
Oral Anticoagulants (e.g., Warfarin)
May increase prothrombin time (PT) and INR, increasing the risk of bleeding. Close monitoring is recommended.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children. Reconstituted suspension should be stored in a refrigerator and used within 7-14 days (check specific product label).

In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Treatment should be symptomatic and supportive.

Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, use in pregnant women should be only if clearly needed. Excreted in very small amounts into breast milk; caution should be exercised when administered to a nursing mother.
