Overview of the medicine
Azithromycin is a macrolide antibiotic used to treat various bacterial infections, including respiratory tract infections, skin infections, and sexually transmitted diseases.
Uses & Indications
Dosage
Adults
For most infections: 500 mg (2 tablets) once daily for 3 days OR 500 mg (2 tablets) on Day 1, then 250 mg (1 tablet) once daily on Days 2-5. For uncomplicated Chlamydia: 1 g (4 tablets) as a single dose.
Elderly
Similar to adults, no specific dosage adjustment required unless severe hepatic or renal impairment is present.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Use with caution in severe impairment.
How to Take
Oral administration, can be taken with or without food.
Mechanism of Action
Azithromycin works by inhibiting bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby stopping bacterial growth.

Onset
Effective antibacterial concentrations are reached within 2-3 hours.
Excretion
Primarily excreted via the bile, with a small portion excreted renally.
Half life
Long, with a terminal elimination half-life of 2-4 days.
Absorption
Rapidly absorbed, with an oral bioavailability of approximately 37%.
Metabolism
Partially metabolized in the liver.
Side Effects
Contraindications
- •Hypersensitivity to azithromycin or any other macrolide antibiotic.
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Antacids
Antacids can reduce the peak plasma concentration of azithromycin; administer azithromycin at least 2 hours before or 4 hours after antacids.
Warfarin
Azithromycin may potentiate the anticoagulant effects of warfarin, increasing the risk of bleeding. Monitor INR.
QT-prolonging drugs
Coadministration with other drugs known to prolong the QT interval should be avoided.
Storage
Store below 30°C, protect from light and moisture.
Overdose
In case of overdose, gastric lavage and general supportive measures should be employed.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Excreted in breast milk, use with caution.
