Overview of the medicine
Zibac 500 mg Tablet contains Azithromycin, a macrolide antibiotic used to treat various bacterial infections, including respiratory tract infections, skin infections, and sexually transmitted diseases. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
For most infections, 500 mg once daily for 3 days, or 500 mg as a single dose on Day 1, followed by 250 mg once daily on Days 2-5. For Chlamydia infections, a single 1-gram (1000 mg) dose.
Elderly
No dosage adjustment needed for elderly patients with normal renal and hepatic function.
Renal_impairment
Mild to moderate renal impairment (creatinine clearance >10 mL/min) does not require dosage adjustment. No data for severe impairment, caution advised.
How to Take
Can be taken with or without food. However, taking it with food may reduce gastrointestinal side effects.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit of susceptible microorganisms. This binding interferes with the translocation of peptides, thus preventing the growth of bacteria (bacteriostatic effect).

Onset
Clinical improvement usually seen within 2-3 days.
Excretion
Mainly excreted unchanged in the bile; a smaller portion is excreted in urine.
Half life
Terminal half-life is 2-4 days (48-96 hours), allowing for once-daily dosing and shorter treatment durations.
Absorption
Rapidly absorbed from the gastrointestinal tract; however, bioavailability is approximately 37%.
Metabolism
Undergoes hepatic metabolism, but to a limited extent; primarily excreted as unchanged drug.
Side Effects
Contraindications
- •Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or any excipient of the tablet.
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
May increase digoxin levels; monitor digoxin levels.
Antacids
Can decrease peak plasma levels of azithromycin; administer azithromycin at least 1 hour before or 2 hours after antacid.
Warfarin
Monitor INR due to potential potentiation of anticoagulant effects.
Nelfinavir
May increase azithromycin serum concentrations.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms may include severe nausea, vomiting, diarrhea, and reversible hearing loss. Gastric lavage and general supportive measures are indicated. No specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Excreted in breast milk, caution advised. Consult a doctor.
