Description
Overview of the medicine
Azithromycin is a macrolide antibiotic used to treat a wide variety of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
500 mg IV as a single daily dose for 1 to 2 days, followed by oral azithromycin once daily to complete a 7- to 10-day course. Administer as an intravenous infusion over 60 minutes.
Elderly
No specific dosage adjustment is required for elderly patients with normal renal and hepatic function.
Renal_impairment
No dosage adjustment is needed for patients with mild to moderate renal impairment (creatinine clearance > 10 mL/min). Use with caution in severe impairment.
How to Take
Administer Zibac 500 mg Injection as an intravenous infusion over 60 minutes. Do not give as a bolus or intramuscular injection.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit of susceptible microorganisms, thereby interfering with the translocation of peptides.

Onset
Rapid for IV administration.
Excretion
Primarily excreted in the bile as unchanged drug, with a small portion excreted renally.
Half life
Prolonged terminal elimination half-life of 2-4 days due to extensive tissue uptake and slow release.
Absorption
Rapidly distributed into tissues; 100% bioavailability for IV form.
Metabolism
Limited hepatic metabolism.
Side Effects
Contraindications
- •Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or to any component of the formulation.
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
May increase serum digoxin levels; monitor digoxin levels.
Antacids
May decrease peak plasma concentrations of azithromycin; administer azithromycin at least 1 hour before or 2 hours after antacids.
Warfarin
Increased anticoagulant effects, monitor INR.
Nelfinavir
Increased azithromycin serum concentrations; no dosage adjustment needed.
Cyclosporine
Potential for elevated cyclosporine levels; monitor cyclosporine levels.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, general symptomatic and supportive measures are indicated as medically necessary. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Azithromycin is excreted in human milk; use with caution in nursing mothers.
