Description
Overview of the medicine
Avloclav 500 mg tablet is a combination antibiotic containing amoxicillin and clavulanic acid. It is used to treat a wide range of bacterial infections that are resistant to amoxicillin alone due to beta-lactamase production.
Uses & Indications
Dosage
Adults
The usual adult dose is one Avloclav 500 mg/125 mg tablet taken three times daily (every 8 hours). Dosage should be adjusted based on the severity of infection and patient's renal function.
Elderly
No specific dose adjustment is generally required for elderly patients unless there is evidence of renal impairment.
Renal_impairment
Dose adjustment is necessary for patients with renal impairment. For CrCl 10-30 mL/min, 500 mg/125 mg every 12 hours. For CrCl <10 mL/min, 500 mg/125 mg every 24 hours. Hemodialysis patients should receive 500 mg/125 mg every 24 hours, with an additional dose given during and at the end of dialysis.
How to Take
Take the tablet whole with a glass of water. To minimize potential gastrointestinal intolerance, take at the start of a meal. Do not crush or chew.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins. Clavulanic acid is a beta-lactamase inhibitor that prevents the degradation of amoxicillin by bacterial beta-lactamase enzymes, thereby extending amoxicillin's antibacterial spectrum.

Onset
Onset of action typically within 1 hour.
Excretion
Both amoxicillin and clavulanic acid are primarily excreted unchanged in the urine, mainly by tubular secretion and glomerular filtration.
Half life
Amoxicillin: Approximately 1-1.5 hours; Clavulanic acid: Approximately 1 hour.
Absorption
Both amoxicillin and clavulanic acid are well absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are achieved within approximately 1-2 hours.
Metabolism
Amoxicillin undergoes minimal metabolism. Clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- •Hypersensitivity to amoxicillin, clavulanic acid, any penicillin, or any component of the formulation.
- •History of severe hypersensitivity reaction to any other beta-lactam antibiotic (e.g., cephalosporins, carbapenems, monobactams).
- •History of jaundice or hepatic dysfunction associated with amoxicillin/clavulanic acid.
Drug Interactions
Probenecid
Decreases renal tubular secretion of amoxicillin, leading to increased and prolonged blood levels of amoxicillin. Concomitant use is not recommended.
Allopurinol
Increased incidence of skin rash when co-administered with amoxicillin.
Methotrexate
May increase methotrexate toxicity due to reduced renal tubular secretion of methotrexate.
Warfarin and other oral anticoagulants
May prolong prothrombin time (INR). Close monitoring of INR is recommended.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include gastrointestinal symptoms such as stomach pain, vomiting, and diarrhea, as well as electrolyte imbalance. Management is primarily symptomatic and supportive, with attention to fluid and electrolyte balance. Hemodialysis can be used to remove amoxicillin/clavulanate from circulation.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. However, use in pregnancy should be undertaken only if clearly needed. Amoxicillin and clavulanic acid are excreted into breast milk. Exercise caution when administering to a nursing mother.
