Description
Overview of the medicine
Broncomet 4 mg Syrup is a combination medication containing Salbutamol, a bronchodilator, and Guaifenesin, an expectorant. It is used to relieve symptoms of cough, chest congestion, and breathing difficulties associated with various respiratory conditions.
Uses & Indications
Dosage
Adults
Adults: 10 ml (2 teaspoonfuls) 3 times daily or as directed by the physician.
Elderly
Elderly: A lower starting dose may be considered. Follow physician's advice.
Renal_impairment
Use with caution. Dosage adjustment may be required based on physician's assessment.
How to Take
Broncomet syrup should be taken orally. It can be taken with or without food. Use a measuring spoon or cup for accurate dosing.
Mechanism of Action
Salbutamol is a selective beta-2 adrenergic agonist that stimulates beta-2 receptors in the lungs, causing relaxation of bronchial smooth muscle and bronchodilation. Guaifenesin is an expectorant that works by increasing the volume and decreasing the viscosity of secretions in the trachea and bronchi, thereby facilitating the removal of mucus by coughing.
Pharmacokinetics
Onset
Bronchodilation from Salbutamol usually occurs within 5-15 minutes.
Excretion
Both Salbutamol and its metabolites, and Guaifenesin and its metabolites, are primarily excreted in the urine.
Half life
Salbutamol: 4-6 hours; Guaifenesin: approximately 1 hour.
Absorption
Salbutamol is well absorbed from the gastrointestinal tract. Guaifenesin is rapidly absorbed.
Metabolism
Salbutamol is metabolized in the liver to an inactive sulfate conjugate. Guaifenesin is rapidly metabolized to beta-(2-methoxyphenoxy) lactic acid.
Side Effects
Contraindications
- Hypersensitivity to Salbutamol, Guaifenesin, or any component of the formulation.
- Patients with hyperthyroidism (relative contraindication for Salbutamol).
Drug Interactions
Digoxin
May alter digoxin levels. Monitor plasma digoxin levels.
Diuretics
Increased risk of hypokalemia with concomitant use.
Beta-blockers
May reduce the effect of Salbutamol and cause severe bronchospasm in asthmatic patients.
MAOIs/Tricyclic Antidepressants
Increased risk of cardiovascular effects.
Storage
Store in a cool and dry place, below 30°C. Protect from light. Keep out of reach of children.
Overdose
Symptoms of Salbutamol overdose include tachycardia, tremors, palpitations, hyperglycemia, and hypokalemia. Guaifenesin overdose symptoms are usually mild gastrointestinal upset. Management involves symptomatic and supportive treatment, including cardiac monitoring if necessary.
Pregnancy & Lactation
Pregnancy Category C for Salbutamol. Use only if potential benefit outweighs potential risk to the fetus. It is unknown if Salbutamol or Guaifenesin is excreted in breast milk. Use with caution during lactation and only if clearly needed after consulting a doctor.
