Description
Overview of the medicine
Cefaclor is a cephalosporin antibiotic used to treat various bacterial infections, including those of the respiratory tract, urinary tract, and skin.
Uses & Indications
Dosage
Adults
250-500 mg every 8 hours, depending on the severity and type of infection. Maximum 4 g/day.
Elderly
Similar to adult dosage, but renal function should be assessed and dosage adjusted if necessary.
Renal_impairment
Dosage adjustment required for severe impairment (e.g., CrCl < 10 mL/min). A loading dose may be given, followed by reduced maintenance doses.
How to Take
Take orally with or without food. Complete the full prescribed course.
Mechanism of Action
Cefaclor inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Onset of action varies by infection type, generally within hours of administration.
Excretion
Primarily excreted unchanged by the kidneys in urine via glomerular filtration and tubular secretion.
Half life
Approximately 0.6-1 hour.
Absorption
Rapidly and well absorbed from the GI tract. Food may delay absorption but does not significantly alter the total amount absorbed.
Metabolism
Not significantly metabolized in the body.
Side Effects
Contraindications
- Known hypersensitivity to cefaclor or any other cephalosporin antibiotics.
- History of severe hypersensitivity reaction to penicillin.
Drug Interactions
Metformin
Cefaclor may increase plasma levels of metformin.
Probenecid
Increases cefaclor plasma concentrations and prolongs its half-life by inhibiting renal excretion.
Aminoglycosides
Although rare, concurrent use with some cephalosporins may increase the risk of nephrotoxicity.
Warfarin and oral anticoagulants
May enhance the anticoagulant effect, requiring monitoring of INR.
Storage
Store in a cool, dry place below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, and diarrhea. Management includes gastric lavage, activated charcoal, and supportive care. Hemodialysis may be useful in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Cefaclor crosses the placenta but is not associated with adverse fetal outcomes. Use with caution during lactation; small amounts are excreted in breast milk.

- Renal function tests (for prolonged therapy or in patients with pre-existing renal impairment)
- Liver function tests (rarely indicated, but consider in patients with hepatic dysfunction)
Doctor Notes
- Confirm bacterial etiology before prescribing.
- Inquire about penicillin allergy history due to potential cross-reactivity.
